Narrative imagination in milieu therapy: Staff members’ stories of relational change

ABSTRACT

Milieu therapy is an occupation-based treatment aimed at enhancing recovery in mental health patients that is grounded in the healing function of being and doing habitual and creative occupations together, thus transcending the individual. The purpose of this study was to explore these situated relations, discovering how relations emerge and are facilitated in the occupational context of a milieu therapy ward. The study is part of an ongoing ethnography published elsewhere. Narrative interviews with one occupational therapist, two nurses and a social worker were conducted on the ward. Data were interpreted through narrative analysis identifying occupation-based relational communication as the main finding. In particular, it was identified that the staff used narrative imagination to facilitate change in the way patients made meaning in the everydayness of milieu therapy. The findings present several relational encounters in which processes of narrative imagination hold significance for patients’ mental health: Welcoming a devastated woman; Solving a mess together; From catastrophe to common ground, and Being at ease with one another. All findings are grounded in occupation-based relations, and the transformative potential of relational occupations is discussed in the milieu therapeutic context.

La terapia ambiental constituye un tratamiento basado en la ocupación tendiente a fomentar la recuperación en pacientes de salud mental. Dicha terapia se fundamenta en la función sanadora que conlleva ser y hacer colectivamente ocupaciones habituales y creativas, trascendiendo lo individual. El presente estudio se orientó a examinar estas relaciones situadas y a dar cuenta de cómo emergen y son facilitadas en el contexto ocupacional de un pabellón de terapia ambiental. Dicho estudio forma parte de una etnografía continua que se publica en otros medios. Con este objetivo, en el pabellón se realizaron entrevistas narrativas con una terapeuta ocupacional, dos enfermeras y una trabajadora social. Los datos aportados por el estudio fueron interpretados empleando el análisis narrativo, determinándose que el principal hallazgo tiene que ver con la comunicación relacional basada en la ocupación. En particular se identificó que el personal usó la imaginación narrativa para facilitar cambios en la manera en que los pacientes dan sentido a la cotidianidad de la terapia ambiental. Los resultados presentan varios encuentros relacionales en que los procesos de imaginación narrativa tienen significación para la salud mental de los pacientes: Dando la bienvenida a una mujer desolada; Resolviendo juntos un problema; De la catástrofe al terreno común; y Sentirnos cómodos entre nosotros. Todos estos hallazgos se sustentan en relaciones basadas en la ocupación; además se discute el potencial transformador que conllevan las ocupaciones relacionales en el contexto de la terapia ambiental.
