Conversion Gait Disorder—Meeting Patients in Behaviour, Reuniting Body and Mind

Abstract

The Hospital for Rehabilitation, Stavern, in Norway has treated patients with physical symptoms with no organic cause, so called conversion disorder patients, for over a decade. For four years research on the treatment has been carried out. Patients with conversion disorder seem not to fit in traditional somatic hospitals because their patienthood depends upon psychiatric diagnosis. Ironically, they appear not to belong in psychiatric hospitals because of their physical symptoms. The treatment offered these patients at hospitals for rehabilitation is adapted physical activity consisting of behaviour elements such as positive reinforcement of normal function and lack of positive reinforcement at dysfunction. The pedagogical approach is seen as crucial in the successful rehabilitation of the patients. The disorder and treatment can be understood by using theories about the ecstatic body, radical behaviourism and phenomenology. When patients have problems in behaviour concerning both body and mind, it would be natural to employ both in the road to recovery. This article describes the various treatments and discusses them from phenomenological, ethical and philosophical perspectives.

Resumen

El hospital para la rehabilitación Stavern, en Noruega, ha tratado a pacientes con síntomas físicos sin causa orgánica, los llamados pacientes con desorden de conversión, durante más de una década. La investigación sobre el tratamiento ha sido llevada a cabo durante cuatro años. Los pacientes con desorden de conversión parece que no cuadran en los hospitales somáticos tradicionales porque su estado como pacientes depende de un diagnóstico psiquiátrico. Irónicamente, parece que no pertenecen en hospitales psiquiátricos a causa de sus síntomas físicos. El tratamiento que se ofrece a estos pacientes en los hospitales de rehabilitación consiste en una actividad física adaptada de elementos de comportamiento tales como el refuerzo positivo de la función normal y la omisión de refuerzo positivo de la disfunción. El enfoque pedagógico se entiende como crucial a la hora de la rehabilitación con éxito de los pacientes. La enfermedad y su tratamiento pueden ser entendidos por medio de teorías acerca del cuerpo extático, la teoría del comportamiento radical [radical behaviourism], y la fenomenología. Cuando los pacientes tienen problemas de comportamiento en cuanto a su cuerpo y mente, sería natural el implicar ambos en el camino hacia la recuperación. Este artículo describe varios tratamientos y los discute desde perspectivas fenomenológicas, éticas y filosóficas.

Zusammenfassung

Die Reha-Klinik Stavern in Norwegen behandelt seit mehr als einem Jahrzehnt Patienten, die körperliche Symptome ohne organische Grundlage aufweisen, sogenannte Konversionsstörungen. Es wurden über vier Jahre Behandlungsmöglichkeiten erforscht. Patienten mit Konversionsstörungen passen scheinbar nicht in traditionelle somatische Kliniken, da ihre Krankheit psychischer Natur ist. Ironischerweise gehören sie nicht in eine psychiatrische Klinik, da sie körperliche Symptome aufweisen. Die Behandlung, die diesen Patienten in Reha-Kliniken zuteil wird, besteht aus APA (adapted physical activity = Bewegung, Spiel und Sport in Prävention, Rehabilitation und Behinderung), insbesondere aus Elementen zum Verhalten, wie zum Beispiel positive Verstärkung normaler Funktionen und das Weglassen dieser Verstärkung bei Dysfunktionen. Die pädagogische Herangehensweise ist besonders wichtig für eine erfolgreiche Rehabilitation der Patienten. Zum Verständnis und zur Behandlung der Störung werden Theorien über ekstatische Körper, radikalen Behaviorismus und Phänomenologie herangezogen. Wenn bei Patienten Verhaltensprobleme auftauchen, die Körper und Geist betreffen, so erscheint es nur natürlich, beide in den Heilungsprozess einzubeziehen. Dieser Artikel beschreibt die verschiedenen Behandlungsmöglichkeiten und bespricht diese aus phänomenologischer, ethischer und philosophischer Perspektive.
