Neuropathic arthropathy of the elbow in a paediatric patient with myelomeningocele

Abstract

Introduction: This case report describes perhaps the youngest case of neuropathic arthropathy (NA) in an upper extremity documented in a patient with myelomeningocele.

Methods: Case report.

Results: This case report describes presenting symptoms, results of radiologic work-up, treatment approach and functional outcome thus far. Included is a clinical picture of the patient as well as a radiograph of the affected elbow.

Discussion: NA, also known as Charcot joint, is a rare occurrence in the upper extremity. NA is well documented in the literature; however, the majority of reports are of adults with affected lower extremities. This report describes the very rare occurrence of upper extremity involvement of a 5 year old child with myleomeningocele. As the literature does not guide treatment for this age of patient, this study reports the treatment plan and the child's outcome thus far. Due to the rarity of this situation, future research will be quite difficult. Reports of cases such as this including treatment and outcome will be helpful to guide clinical decision-making for future cases.

Artropatía Neuropática del codo en un paciente pediátrico con Mielomeningocele. Introducción: Este reporte de un caso describe probablemente el caso más joven de Artropatía Neuropática (AN) en una extremidad superior documentado en un paciente con Mielomeningocele. Métodos: Reporte de un caso. Resultados: Este reporte de un caso describe los síntomas, resultados radiológicos, estrategias de manejo y el resultado funcional y clínico hasta el momento. Se incluyen imágenes clínicas así como radiográfias del codo afectado. Discusión: La AN, también conocida como Articulación de Charcot es raro que se presente en la extremidad superior. La AN se encuentra bien documentada en la literatura, sin embargo la mayoría de los reportes son de adultos con afección en las extremidades inferiores. Este reporte describe la rara existencia de AN en la extremidad superior en un niño de 5 años con Mielomenigocele. La literatura no ofrece guías de tratamiento en pacientes con esta edad, por lo que reportamos nuestro plan de tratamiento y el resultado obtenido en el niño hasta el momento. Debido a lo raro de la situación, será defícil realizar investigaciones a futuro. Los reportes de caso como éste en el que se incluye el tratamiento y el resultado, serán útiles como una guía clínica para la toma de decisiones en casos futuros.
