An atypical life-threatening presentation, with delayed diagnosis, in a 22-year-old female

Abstract

Acute lupus pneumonitis (ALP) is a rare life-threatening complication in systemic lupus Erythematosus (SLE) that may present with nonspecific respiratory symptoms. Typically, ALP presents following the formal diagnosis of SLE. Here we present the first case in North America, to our knowledge, of acute lupus pneumonitis as the initial presentation of SLE, with no prior respiratory symptoms and only one ACR (American College of Rheumatology) criteria fulfilled for SLE diagnosis.

RÉSUMÉ

La pneumonie lupique aiguë est une complication rare du lupus érythémateux systémique (LES) susceptible d’engager le pronostic vital qui peut présenter des symptômes respiratoires non spécifiques. Habituellement, la PLA fait son apparition après le diagnostic officiel de LES. Nous présentons ici le premier cas en Amérique du Nord, à notre connaissance, de pneumonie lupique aiguë en tant que présentation initiale du LES, sans symptômes respiratoires antérieurs et ne répondant qu’à un seul critère de l’ACR (American College of Rheumatology) pour le diagnostic de LES.
