Abstract
RATIONALE AND OBJECTIVE: The snoring, tiredness, observed apnea, high BP, BMI, age, neck circumference, and male gender (STOP-Bang) questionnaire is used widely to screen individuals at high risk of OSA. The objective of the study is to examine the diagnostic performance of the STOP-Bang questionnaire versus the STOP-Bag (without neck circumference) questionnaire. We hypothesized that the diagnostic performance of the STOP-Bang questionnaire would be higher than STOP-Bag questionnaire.
METHODS: A retrospective study was conducted that included patients from two preoperative clinics. All participants completed the STOP-Bang questionnaire and underwent polysomnography (PSG). The diagnostic parameters were calculated for the STOP-Bang questionnaire and the STOP-Bag questionnaire versus polysomnography as the reference standard.
RESULTS: There were 203 patients with mean age of 57 ± 13 years and 51% were male. The STOP-Bang questionnaire had a significantly higher area under receiver operating curve than the STOP-Bag questionnaire (0.782 vs 0.758, P < 0.05) in detection of mild to severe OSA in surgical patients. Similarly, the STOP-Bang questionnaire had significantly higher sensitivity when compared to the STOP-Bag questionnaire (85.5% vs 81.3%, P < 0.05). The area under the curve for screening moderate-to-severe and severe OSA was not significantly different for STOP-Bang and STOP-Bag questionnaires.
CONCLUSION: Compared to the STOP-Bag questionnaire, the STOP-Bang questionnaire has higher diagnostic performance in predicting all OSA, but the 2 questionnaires were similar for moderate-to-severe and severe OSA. The STOP-Bag questionnaire can be used for screening OSA when neck circumference measurement is not feasible.
RÉSUMÉ
JUSTIFICATION ET OBJECTIF: Le questionnaire STOP-Bang est largement utilisé pour dépister les personnes à haut risque d’apnée obstructive du sommeil. L’objectif de l’étude est d’examiner la performance diagnostique du questionnaire STOP-Bang par rapport au questionnaire STOP-Bag (sans circonférence du cou). Nous avons émis l’hypothèse que la performance diagnostique du questionnaire STOP-Bang serait supérieure à celle du questionnaire STOP-Bag.
MÉTHODES: Une étude rétrospective a été menée auprès de patients de deux cliniques préopératoires. Tous les participants ont rempli le questionnaire STOP-Bang [S : Ronflement, T : Fatigue, O : Apnée observée, P : Tension artérielle, B : IMC (> 35 kg/m2), a : âge (> 50 ans), n : circonférence du cou (> 40 cm), et g : sexe (homme)] et ont subi une polysomnographie. Les paramètres diagnostiques ont été calculés pour le questionnaire STOP-Bang et le questionnaire STOP-Bag et comparés à la polysomnographie comme étalon de référence.
RÉSULTATS: Il y avait 203 patients avec un âge moyen de 57 ± 13 ans et 51% étaient des hommes. Le questionnaire STOP-Bang présentait une surface sous la courbe de fonctionnement du récepteur significativement plus élevée que le questionnaire STOP-Bag (0,782 comparativement à 0,758, P < 0,05) dans la détection d’une apnée obstructive du sommeil légère à sévère chez les patients chirurgicaux. De même, le questionnaire STOP-Bang présentait une sensibilité significativement plus élevée par rapport au questionnaire STOP-Bag (85,5 % comparativement à 81,3 %, p < 0,05). La zone sous la courbe pour le dépistage de l’apnée obstructive du sommeil modérée à sévère et sévère n’était pas significativement différente pour les questionnaires STOP-Bang et STOP-Bag.
CONCLUSION Comparativement au questionnaire STOP-Bag, le questionnaire STOP-Bang a une performance diagnostique plus élevée dans la prédiction de toutes les apnées du sommeil, mais les deux questionnaires étaient similaires pour les apnées obstructives du sommeil modérées à sévères et sévères. Le questionnaire STOP-Bag peut être utilisé pour le dépistage de l’apnée obstructive du sommeil lorsque la mesure de la circonférence du cou n’est pas possible.
