The sense of the body in the dream: Diagnostic capacity in the meanings of dreams

The author investigates the oneiric representation of somatic states and the diagnostic capacity of dreams. He draws on Freud's hypotheses on the procedures by which somatic stimuli insert themselves in oneiric elaboration and restructures them according to the recent neurobiological discoveries and to analytical experiences. In the representations of certain dreams, with a psychic interpretation agreed upon by the patients, somatic alterations unknown to the analytical couple were discriminated and confirmed by radiological investigations. These representations were linked to the manifestation of one aspect of the bodily Self, neglected in the precocious maternal relation, that entered the organization of the Self consolidated in the relation with the paternal figure. This conjunction gave origin to the double meaning (somatic and psychic) of the dream. The entering of the somatic representation in the oneiric one did not appear to be the figurative effect, but of a condensation of diagnostic capacity into the meaning of the dream. This characteristic manifested itself in the particular styles of the dreamers, interpretable by an analyst countertransferentially oriented. The perception or scotomization of the condensation in the interpretation of the dream and of the moment had an effect on the evolution of the analysis.

Les sensations corporelles dans le rêve, le pouvoir diagnostique du sens des rêves

L'auteur de cet article explore la représentation onirique des états somatiques et le pouvoir diagnostique des rêves. Il s'inspire des hypothèses de Freud sur les processus via lesquels les stimuli somatiques s'introduisent dans l'élaboration onirique, tout en les reformulant selon les découvertes neurobiologiques récentes et l'expérience analytique. Certains rêves, dont l'interprétation psychique a été admise par les patients, contiennent des représentations afférentes à des changements somatiques décelés et confirmés par des investigations radiologiques, mais dont le couple analytique n'avait pas eu connaissance préalablement. Ces représentations étaient liées à la manifestation d'un aspect du self corporel qui, laissé de côté dans la relation maternelle précoce, aura pu s'introduire dans l'organisation du Self, une fois celui‐ci consolidé du fait de la relation à une figure paternelle. Cette conjonction est à l'origine d'un double sens (somatique et psychique) du rêve. L'introduction de la représentation somatique dans la représentation onirique n'apparaît pas comme l'effet d'une figuration, mais comme celui d'une condensation du pouvoir diagnostique pénétrant le sens du rêve. Cette caractéristique, qui trouve son expression dans le style propre aux rêveurs, est interprétable par un analyste orientant son écoute du côté du contre‐transfert. Der Autor untersucht die Repräsentation somatischer Zustände im Traum und die diagnostische Fähigkeit von Träumen. Er stützt sich auf Freuds Hypothesen über die Art und Weise, wie somatische Stimuli in die Traumbildung eingehen, und modifiziert sie gemäß aktuellen neurobiologischen Entdeckungen und analytischen Erfahrungen. In den Darstellungen bestimmter Träume, deren psychischer Deutung die Patienten zustimmten, wurden somatische Veränderungen, die dem analytischen Paar zuvor unbekannt waren, identifiziert und durch radiologische Untersuchung bestätigt. Diese Darstellungen hingen mit der Manifestation eines bestimmten Aspekts des Körperselbst zusammen, der in der frühreifen Beziehung zur Mutter vernachlässigt worden und in die Organisation des Selbst, so wie sie sich in der Beziehung zur Vaterfigur konsolidierte, eingegangen war. Diese Verbindung lag der doppelten (somatischen und psychischen) Bedeutung des Traumes zugrunde. Dass die somatische Repräsentation in die Traumrepräsentation einging, war kein Ergebnis der Traumbilder, sondern resultierte aus der Verdichtung der diagnostischen Fähigkeit zur Bedeutung des Traumes. Diese Besonderheit manifestierte sich in dem spezifischen Stil der Träumenden, der von dem an seiner Gegenübertragung orientierten Analytiker gedeutet werden konnte. Die Wahrnehmung oder Skotomisierung dieser Verdichtung in der Deutung des Traumes und des Moments beeinflusste die weitere Entwicklung der Analyse.

Il senso del corpo e la capacità diagnostica del sogno contenuti nei significati del sogno

L'autore esamina la rappresentazione onirica di stati somatici, nonché la capacità diagnostica del sogno. L'autore parte dalle ipotesi di Freud sulle procedure per mezzo delle quali gli stimoli somatici si inseriscono nell'elaborazione onirica, e le riorganizza secondo le recenti scoperte neurobiologiche ed esperienze analitiche. Nelle rappresentazioni di alcuni sogni, le interpretazioni psichiche proposte dall'analista e accolte dai pazienti hanno rivelato alterazioni somatiche precedentemente ignorate dalla coppia analitica, che esami radiologici successivi hanno individuato e confermato.

Queste rappresentazioni erano collegate alla manifestazione di un aspetto del Sé corporeo trascurato dalla relazione precoce con la madre, che sono entrate nell'organizzazione del Sé consolidatasi nella relazione con la figura paterna. Questa congiunzione ha dato origine al doppio significato (somatico e psichico) del sogno. L'infiltrazione della rappresentazione somatica in quella onirica non è parsa essere dovuta all'effetto figurativo, ma ad una condensazione della capacità diagnostica all'inteno del significato del sogno. Questa caratteristica si manifesta in special modo nello stile di chi sogna, interpretabile dall'analista orientato dall'ascolto del controtransfert. La percezione o lo scotomizzazione della condensazione nell'interpretazione del sogno e del momento ha avuto un effetto sull'evoluzione dell'analisi.

El autor investiga la representación onírica de los estados somáticos y la capacidad diagnóstica de los sueños. Para ello, se basa en las hipótesis de Freud sobre los procedimientos mediante los cuales los estímulos se insertan en la elaboración onírica, y las reelabora de acuerdo con hallazgos recientes en el campo de la neurobiología y con sus experiencias clínicas. En las representaciones de ciertos sueños, a través de una interpretación psíquica aceptada por los pacientes, se discriminaron alteraciones somáticas que la pareja analítica desconocía y que fueron confirmadas por el diagnóstico por imágenes. Estas representaciones estaban ligadas a la manifestación de un aspecto del sí mismo corporal, descuidado en la relación materna temprana, que penetró la organización del sí mismo consolidado en la relación con la figura paterna.

Esta conjunción dio origen al doble sentido (somático y psíquico) del sueño. La penetración de la representación somática en la onírica no parecía corresponder al efecto figurativo, sino a una condensación de la capacidad diagnóstica en el significado del sueño. Dicha característica se manifestó en los estilos particulares de los soñantes, que resultaron interpretables a través de la exploración de la contratransferencia. La percepción de la escotomización de la condensación en la interpretación del sueño y del momento afectó la evolución del análisis.
