1. a Daryl Guest was a young optometrist who graduated in 1977 and just completed a Master's degree. He would later be Deputy Clinic Director at the Victorian College of Optometry, an innovative private practitioner in Tasmania, a much sought after speaker for continuing education and is now Associate Professor and director of the Optometry Clinic in the University of Melbourne.
2. b This procedure for setting medical and optometric fees based on changing practice costs continued until the early 1990s, when the medical associations refused to co‐operate and the government of the day adjusted the fees by Wage Cost Index 5, a very conservative estimate of inflation with no real relationship to changing practice costs, a situation which continues to this day.
3. c Dr Kiely was appointed Research Officer at OAA in 1991.
