Examples of social determinants of health (SDoH) are given for each hierarchy (rows in pyramid) and system level (columns in pyramid) and are not meant to be inclusive of all social needs. In following the hierarchy, lower-level SDoH will take priority in terms of an individual’s motivation towards diabetes health over higher level SDoH.
