Notes: Hypertension was defined as treated with antihypertensive drugs or office blood pressure >140/90 mmHg. Dyslipidemia was defined as treated with lipid-lowering drugs. Diabetes was defined as undertreatment for diabetes. Physically active was defined as when a subject exercised at least 30 minutes a day more than three times a week, moderately active was when a subject exercised at least 30 minutes once a week, and inactive was no activity over a week.
