Hypnotherapy in Management of Pain and Reexperiencing of Trauma in Burn Patients

Abstract

This study examined the effects of hypnosis on both pain and reexperiencing of trauma in burn patients. Forty-four patients hospitalized for burn care were randomly assigned to either hypnotherapy or a control group. Direct and indirect hypnotic suggestions were used to reduce pain and reexperiencing of trauma. All patients received routine burn care. Pain reports were quantified by using a self-report numeric rating scale ranging from 0 to 5. The number of recalled vivid, troubling events of the trauma in 24-hour intervals was used for rating the reexperiencing of trauma. The hypnotherapy group showed significantly lower pain ratings than the control group and reported a significant reduction in pain from baseline. There was a significant reduction in trauma reexperience scores in the hypnotherapy group but not the control group. The findings support the efficacy of hypnotherapy in the management of both pain and reexperiencing of trauma in burn patients.

Zusammenfassung

Diese Studie untersuchte die Auswirkungen von Hypnose auf Schmerz und Traumawiedererleben bei Verbrennungspatienten. 44 Patienten, die Krankenhausaufenthalte infolge von Verbrennungen hinter sich hatten, wurden zufällig einer Hynosetherapie- und einer Kontrollgruppe zugewiesen. Direkte und indirekte hypnotische Suggestionen wurden eingesetzt, um die Schmerzen zu reduzieren und das Wiedererleben des Traumas zu vermindern. Alle Patienten erhielten zusätzlich die medizinische Standardbehandlung bei Verbrennungen. Die Berichte von Schmerzen wurden über eine numerische Selbstberichtsskala mit 5 Abstufungen quantifiziert. Die Hypnosetherapiegruppe zeigte eine signifikant geringere Schmerzbewertung als die Kontrollgruppe und berichtete eine signifikante Reduktion des Schmerzes gegenüber der Baseline. Es zeige sich eine signifikante Reduktion des Wiedererlebens des Traumas in der Hypnosetherapiegruppe, nicht aber in der Kontrollgruppe. Die Befunde belegen die Wirksamkeit von Hypnosetherapie bei der Behandlung von Schmerz und traumatischem Wiedererleben.

Cette étude a cherché à connaître les effets de l'hypnose sur la douleur et sur le souvenir d'un traumatisme chez des grands brûlés. Quarante-quatre patients hospitalisés pour des brûlures ont été sélectionnés au hasard, soit comme candidats à l'hypnothérapie, soit comme témoins. Des suggestions hypnotiques directes et indirectes ont été utilisées pour soulager la douleur et effacer le souvenir du traumatisme. Tous les patients ont reçu les soins habituels pour grands brûlés. La douleur était quantifiée selon une échelle numérique autocotée allant de 0 à 5. Le nombre de souvenirs troublants, très nets, du traumatisme, enregistré par intervalles de 24 heures, a été utilisé pour quantifier l'évocation du traumatisme. Le groupe ayant suivi l'hypnothérapie a signalé avoir ressenti des douleurs considérablement moindres que le groupe témoin, ainsi qu'une réduction significative de la douleur comparativement à la ligne de base. On a signalé une réduction notable de l'évocation du traumatisme au sein du groupe ayant suivi l'hypnothérapie, mais aucune chez le groupe témoin. Ces résultats soutiennent l'efficacité de l'hypnothérapie dans la gestion de la douleur et de l'évocation d'un traumatisme chez les grands brûlés.

Resumen

Este estudio examinó los efectos de la hipnosis en el dolor y re-experimentación de trauma en pacientes de quemadura. Asignamos aleatoriamente a 44 pacientes hospitalizados para el cuidado de quemadura a hipnoterapia o un grupo control. Usamos sugestiones hipnóticas directas e indirectas para reducir dolor y re-experimentación de trauma. Todos los pacientes recibieron el cuidado de quemadura de rutina. Evaluamos los informes de dolor mediante una escala numérica de auto-informe de 0 a 5. Usamos el número de recuerdos vívidos e inquietantes del trauma en intervalos de 24 horas para puntuar la re-experimentación del trauma. El grupo de hipnoterapia mostró puntuaciones de dolor significativamente más bajas que el grupo control y mencionó una reducción significativa del dolor de la línea base. Hubo una reducción significativa en la re-experimentación de trauma en el grupo de hipnoterapia pero no en el grupo control. Los resultados apoyan la eficacia de la hipnoterapia en el tratamiento de dolor y re-experimentación de trauma en pacientes de quemadura.
