Effects of a Hypnotic Induction and an Unpleasantness-Focused Analgesia Suggestion on Pain Catastrophizing to an Experimental Heat Stimulus: A Preliminary Study

Abstract

Pain catastrophizing is associated with greater levels of pain. While many studies support the efficacy of hypnosis for pain, the effect on pain catastrophizing remains unclear. The present study evaluated the effect of hypnosis on pain catastrophizing using experimental heat stimulation. Twenty-two pain patients engaged in 3 conditions: baseline (no suggestion), hypnotic induction, and hypnotic induction plus analgesia suggestion. Participants with higher baseline pain showed a significant reduction in rumination following hypnotic induction plus analgesia suggestion and significant reductions in pain due to both the hypnotic induction alone and the hypnotic induction plus analgesia suggestion. The findings suggest that unpleasantness-focused hypnotic analgesia reduces pain via its effect on the rumination component of pain catastrophizing.

Abstract

Die Katastrophisierung von Schmerz ist mit höheren Schmerzleveln assoziiert. Während viele Studien die Effizienz von Hypnose bei Schmerzen unterstützen, bleibt der Effekt auf die Katastrophisierung von Schmerz unklar. Die aktuelle Studie untersuchte den Effekt von Hypnose auf die Katastrophisierung von Schmerz mittels experimenteller Hitzestimulation. 22 Schmerzpatienten wurden 3 Bedingungen zugeordnet: Baseline (keine Suggestion), hypnotische Induktion und hypnotische Induktion plus Suggestion einer Analgesie. Teilnehmer mit höherem baseline Schmerz zeigten eine signifikante Reduktion der Rumination auf hypnotische Induktion plus Suggestion einer Analgesie und signifikante Reduktion von Schmerz infolge der hypnotischen Induktion allein und der hypnotischen Induktion plus Suggestion einer Analgesie. Die Ergebnisse lassen vermuten, daß eine Unannehmlichkeiten-fokussierte hypnotische Analgesie den Schmerz mittels ihres Effektes auf die Ruminationskomponente der Schmerzkatastrophisierung vermindern kann.

La dramatisation de la douleur est associée à des niveaux supérieurs de douleur. Si de nombreuses études confirment l’efficacité de l’hypnose pour soulager la douleur, l’effet que peut avoir la dramatisation de la douleur sur celle-ci demeure difficile à déterminer. Cette étude a permis d’évaluer l’effet de l’hypnose sur la dramatisation de la douleur à l’aide d’une stimulation thermique expérimentale. Vingt-deux patients éprouvant des douleurs ont été répartis en trois groupes: un groupe de base (absence de suggestion), un groupe avec induction hypnotique, et un groupe avec induction hypnotique et suggestion d’analgésie. Les participants ayant éprouvé les plus fortes douleurs au début de l’étude ont signalé une réduction significative de leurs ruminations après l’induction hypnotique avec suggestion d’analgésie, et une réduction significative de leurs douleurs à la suite de l’induction hypnotique seule et de l’induction hypnotique avec suggestion d’analgésie. Ces résultats semblent indiquer que l’analgésie hypnotique axée sur le malaise réduit la douleur par son effet sur l’aspect rumination de la dramatisation de la douleur.

Resumen

Catastrofizar el dolor está asociado con mayores niveles de dolor. Mientras muchos estudios sustentan la eficacia de la hipnosis para el dolor, el efecto sobre la catastrofización del dolor permanece incierto. El presente estudio evaluó el efecto de la hipnosis en la catastrofización del dolor usando una estimulación experimental de calor. Veintidós pacientes participaron en tres condiciones: línea basal (sin sugerencias), inducción hipnótica, e inducción hipnótica más sugerencias de analgesia. Los participantes con una línea basal de dolor más alta mostraron una reducción significativa en rumiación después de la inducción hipnótica más sugerencias de analgesia y reducciones significativas de dolor debido tanto a la inducción hipnótica por sí sola y la inducción hipnótica más sugerencias de analgesia. Estos hallazgos sugieren que la analgesia hipnótica enfocada a lo desagradable reduce el dolor a través de sus efectos en el componente de rumiación en la catastrofización del dolor.
