EFFECTS OF CONVERSATIONAL HYPNOSIS ON RELATIVE PARASYMPATHETIC TONE AND PATIENT COMFORT DURING AXILLARY BRACHIAL PLEXUS BLOCKS FOR AMBULATORY UPPER LIMB SURGERY:A Quasiexperimental Pilot Study

Abstract

This two-center quasiexperimental pilot study was to determine the effect of conversational hypnosis on patient comfort and parasympathetic tone, which may represent a quantitative measure of hypnotic depth, during regional anesthesia. The patients received conversational hypnosis in one center and oral premedication in the other. The patients’ subjective comfort (0-10 rating scale) and objective parasympathetic tone, as assessed by the Analgesia/Nociception Index (ANI), were measured before and after regional anesthesia. The parasympathetic tone and comfort scores evidenced a significantly greater increase in the hypnosis patients than in controls. These findings suggest that using conversational hypnosis during regional anesthesia may be followed by a subjective increase in patient comfort and an objective increase in parasympathetic tone, monitored by ANI.

Abstract: Diese zweizentrische quasiexperimentelle Pilotstudie sollte den Effekt konversationeller Hypnose auf das Patientenbefinden und den parasympathischen Tonus aufzeigen, welche ein quantitatives Maß für die Hypnosetiefe während regionaler Anästhesie sein könnten. Die Patienten erhielten konversationelle Hypnose in dem einen Zentrum und eine orale Prämedikation in dem anderen. Das subjektive Patientenbefinden (auf einer Skala von 1-10) und der objektive parasympathische Tonus, welcher mittels Analgesia/Nociception Index (ANI) aufgenommen wurde, wurden vor und nach der Regionalanästhesie gemessen. Der parasympathische Tonus und die Zahlen zum Patientenbefinden bewiesen einen signifikant größeren Anstieg bei den hypnotisierten Patienten als in der Kontrollgruppe. Diese Ergebnisse lassen vermuten, daß die Anwendung konversationeller Hypnose während einer Regionalanästhesie von einem subjektiv besseren Patientenbefinden und einem objektiven Anstieg des parasympathischen Tonus, mittels ANI gemessen, gefolgt werden könnten.

Résumé: Cette étude pilote quasiexpérimentale menée dans deux centres visait à déterminer l’effet de l’hypnose conversationnelle sur le confort du patient et le tonus parasympathique relatif, pouvant représenter une mesure quantitative de la profondeur de l'hypnose pendant une anesthésie régionale. Dans le premier centre, les patients bénéficié d'une séance d’hypnose conversationnelle et dans le second, ils ont reçu une prémédication orale. Le niveau de confort subjectif des patients (échelle numérique de confort de 0 à 10) et le tonus parasympathique objectif, évalué par par l’Indice Analgésie/Nociception (ANI), ont été mesurés avant et après l’anesthésie régionale. Les scores de tonus parasympathique et de confort ont montré une augmentation significative du confort chez les patients hypnotisés que chez ceux du groupe témoin. Ces résultats semblent indiquer que l’utilisation de l’hypnose conversationnelle pendant une anesthésie régionale peut entraîner une augmentation subjective du confort du patient et une augmentation objective du tonyus parasympathique, monitorées par l’ANI.

Resumen: Este estudio piloto cuasiexperimental en dos centros se realizó para determinar el efecto de la hipnosis conversacional sobre la comodidad del paciente y el tono parasimpático, que podría representar una medición cuantitativa de la profundidad hipnótica durante anestesia regional. Los pacientes recibieron hipnosis conversacional en un centro y medicamentos orales previos en el otro. Se evaluaron, antes y después de la anestesia regional, el nivel subjetivo de comodidad del paciente (en una escala de 0-10) y, objetivamente, el tono parasimpático mediante el Índice de Analgesia/Nocicepción (ANI por sus siglas en Inglés). El tono parasimpático y los niveles de comodidad evidenciaron un incremento significativamente mayor en los pacientes con hipnosis comparados a los controles. Estos hallazgos sugieren que el utilizar la hipnosis conversacional durante la anestesia regional puede anteceder a un incremento subjetivo en el nivel de comodidad del paciente y un aumento objetivo en el tono parasimpático, monitoreado por el ANI.
