Abstract
Dysphagia, the perceived impediment to swallowing food, is a common postsurgical symptom that can have debilitating consequences. This study presents the successful treatment of severe esophageal dysphagia with a problem-focused and patient-centered approach to hypnosis, informed by long-term empirical follow-up. The authors describe a case history involving significant and persistent difficulty swallowing for which no medical or surgical treatment could be found. Over the course of 10 sessions, the patient was assessed, treated with imaginal exposure, and instructed in self-hypnosis. Outcomes were measured at treatment conclusion, and 6-, 9-, and 18-month follow-ups. Following hypnosis, the patient exhibited significant and reliable change (RC) in visceral hypersensitivity (RC = −3.16, p = .002), emotional distress (RC = −2.21, p= .03), subjective well-being (RC = 4.14, p< .0001), and posttraumatic symptoms (RC = −3.33, p= .001). Gains were maintained at 18-month follow-up.

Abstract: Dysphagie, eine empfundene Schluckstörung, ist ein bekanntes postoperatives Symptom das beeinträchtigende Konsequenzen haben kann. Diese Studie präsentiert die erfolgreiche Behandlung schwerer ösophagealer Dysphagie mit einem Problem-fokussierten und Patienten-zentrierten Vorgehen zu Hypnose, basierend auf einem langzeitigen empirischen follow-up. Die Autoren beschrieben eine Fallgeschichte, die eine signifikante und persistierende Schwierigkeit zu schlucken beinhaltet, für die keine medizinische oder chirurgische Behandlung gefunden werden konnte. Über die Zeit von 10 Sitzungen wurde der Patient evaluiert, mit imaginärer Exposition behandelt und in Selbsthypnose instruiert. Die Wirkungen wurden am Ende der Behandlung, nach 6, 9 und 18 Monaten gemessen. Nach der Hypnose zeigte der Patient signifikante und verlässliche Veränderungen (reliable change = RC) bezüglich der viszeralen Hypersensibilität (RC = −3,16, p = 0,002), dem emotionalem Leid (RC = −2,21, p = 0,03), dem subjektivem Wohlbefinden (RC = 4,14, p < 0,0001) und posttraumatischen Symptomen (RC = −3,33, p = 0,001). Die Steigerungen blieben auch am 18-Monate follow-up.

Résumé: La dysphagie, soit la sensation de gêne ou d’obstacle à la déglutition, est un symptôme post-chirurgical courant susceptible d’avoir des conséquences débilitantes. Cette étude présente le cas d’un traitement réussi d’une dysphagie œsophagienne grave grâce à une approche de l’hypnose axée sur le problème et sur le patient, guidée par un suivi empirique à long terme. Les auteurs décrivent une étude de cas mentionnant la présence de difficultés graves et persistantes de déglutition, pour lesquelles il n’existait aucun traitement médical ou chirurgical. Au fil de 10 séances, le patient a été évalué, traité au moyen d’une exposition en imagination, et initié à l’autohypnose. Les résultats ont été mesurés à la fin du traitement, et lors des suivis à 6, 9 et 18 mois. À la suite des interventions hypnotiques, le patient a présenté un changement fiable (CF) et significatif en matière d’hypersensibilité viscérale (CF = −3,16, p = 0,002), de détresse émotionnelle (CF = −2,21, p = 0,03), de bien-être subjectif (CF = 4,14, p < 0,0001) et de symptômes post-traumatiques (CF = −3,33, p = 0,001). Ces gains avaient été maintenus lors du suivi à 18 mois.

Resumen: Disfagia, el impedimento percibido para tragar comida, es un síntoma postquirúrgico común que puede tener consecuencias debilitantes. Este estudio presenta el tratamiento exitoso de una disfagia esofágica severa con un enfoque de hipnosis centrado en soluciones y en el paciente, informado por un seguimiento empírico a largo plazo. Los autores describen una historia clínica relacionada con dificultad persistente y significativa para tragar a la que no se le había encontrado tratamiento médico o quirúrgico. En el curso de 10 sesiones, el paciente fue evaluado, tratado con exposición a visualizaciones e instruido en autohipnosis. Los resultados se evaluaron a la conclusión del tratamiento, y en seguimientos a 6, 9 y 18 meses. Después de la hipnosis, el paciente exhibió un cambio fiable y significativo (RC) en hipersensibilidad visceral (RC = −3.16, p = .002), tensión emocional (RC = −2.21, p = .03), bienestar subjetivo (RC = 4.14, p = <.0001), y síntomas postraumáticos (RC = −3.33, p = .001). Las ganancias se mantuvieron hasta el seguimiento a 18 meses.
