From the talking cure to a disease of silence: Effects of ethical violations in a psychoanalytic institute

ABSTRACT

This article presents an in-depth study of one institute’s efforts to recover from effects of ethical violations by two senior members. Qualitative data analysis from voluntary member interviews details the damage that spread throughout the institute, demonstrating that a violation of one is a violation of many. Members at all levels reported feeling disturbed in ways that affected their emotional equilibrium, their thinking processes, and their social and professional relationships. The aggregated interview data were reported to the institute community in large and small group meetings designed to reverse the “disease of silence” and to allow members to talk with each other. Outside consultation helped with this emotionally arduous process. The authors offer hypotheses concerning the nature of group anxieties during ethics crises. We assert that both sexual and non-sexual boundary violations break the incest taboo, as they breach the generational protection required of professional interactions. Ethical violations attack the group’s foundational ethos of care, unleashing primitive anxieties and defences that interfere with capacities for thinking, containment, collaboration, and integration. Since the full reality of what happened is unknowable, hybrid truths emerge, causing conflict and disturbances that inhibit thoughtful group discourse.

Cet article est consacré à une étude approfondie des efforts entrepris par un de nos instituts pour guérir des effets des violations éthiques commises par deux de ses membres titulaires. Une analyse qualitative de données issues d'entretiens réalisés auprès de membres qui se sont portés volontaires expose en détail les ravages qui se sont répandus à travers tout l'institut, mettant ainsi en évidence que toute violation à l'encontre d'une seule personne et une violation à l'encontre de tous. Quel qu'ait été leur statut hiérarchique, les membres de l'institut ont rapporté qu'ils s'étaient sentis atteints dans leur équilibre émotionnel, leurs processus de pensée et leurs relations sociales et professionnelles. L'ensemble des données issues de ces entretiens a été transmis à la communauté des membres de l'institut, réunis en petits groupes ou en groupes plus élargis, et ce afin de rompre la « maladie du silence » et de permettre aux membres d'échanger les uns avec les autres. Une supervision externe a également contribué à affronter ce processus émotionnel éprouvant. Les auteurs de cet article émettent des hypothèses quant à la nature des angoisses de groupe lors de crises éthiques. Ils affirment que toute violation des frontières, qu'elle soit d'ordre sexuel ou non-sexuel, constitue une transgression du tabou de l'inceste, dans la mesure où elle porte atteinte à la protection générationnelle qui est requise au sein des interactions professionnelles. Les violations à caractère éthique attaquent le groupe dans son fondement éthique du soin, déclenchant alors des angoisses et des défenses primitives qui entravent les capacités de pensée, de contenance, d'élaboration et d'intégration. Étant donné que la réalité pleine et entière de ce qui s'est passé demeure inconnaissable, on assiste à l'émergence de vérités hybrides, qui provoquent des conflits et des perturbations venant inhiber la capacité réflexive du groupe.

Der Beitrag beschreibt eine Studie über die Bemühungen eines Instituts, die Folgen der Ethikverstöße aufzuarbeiten, deren sich zwei leitende Mitglieder schuldig gemacht hatten. Eine qualitative Datenanalyse auf der Grundlage freiwilliger Mitgliederinterviews zeigt detailliert, dass der Schaden das gesamte Institut betraf und dass die Verletzung eines Mitglieds eine Verletzung vieler ist. Alle Mitglieder, ganz gleich, welcher Ebene sie angehörten, berichteten, sich in ihrem emotionalen Gleichgewicht, in ihren Denkprozessen sowie ihren sozialen und professionellen Beziehung beeinträchtigt gefühlt zu haben. Die gesammelten Interviewdaten wurden den Institutsangehörigen in großen und kleinen Gruppenmeetings vorgestellt, um die „Krankheit des Schweigens“ zu behandeln und den Mitgliedern Gelegenheit zu geben, miteinander zu sprechen. Äußere Konsultationen erleichterten diesen emotional belastenden Prozess. Die Autoren formulieren Hypothesen bezüglich der Art der Gruppenängste, die in ethischen Krisen auftauchen. Wir behaupten, dass sowohl sexuelle als auch nicht-sexuelle Grenzverletzungen gegen das Inzesttabu verstoßen, weil sie die Schutzfunktion, die die ältere Generation im Rahmen professioneller Interaktionen erfüllen muss, zunichtemachen. Verstöße gegen die Ethik sind Angriffe auf das grundlegende Gruppenethos der Fürsorge und setzen primitive Ängste und Abwehrmechanismen frei, die wiederum die Fähigkeit zu denken und das Containment, die Kollaboration und Integration beeinträchtigen. Weil die Realität dessen, was geschehen ist, nicht zur Gänze gekannt werden kann, tauchen hybride Wahrheiten auf und verursachen Konflikte und Störungen, die einen nachdenklichen Gruppendiskurs unmöglich machen.

L’articolo presenta uno studio approfondito degli sforzi messi in atto da un istituto psicoanalitico per riprendersi dagli effetti delle violazioni del codice etico commesse da due suoi membri anziani. L’analisi dei dati qualitativi raccolti attraverso interviste su base volontaria ai membri dell’istituto illustra l’entità del danno estesosi poi all’intero istituto, dimostrando come l’abuso di una sola persona sia di fatto l’abuso di molte. I membri di ogni grado hanno riferito di essersi sentiti colpiti dagli eventi in modi che hanno avuto ricadute negative sul loro equilibrio emotivo, sui loro processi di pensiero e sui loro rapporti sociali e professionali. L’aggregato dei dati delle interviste è stato poi restituito alla comunità dell’istituto nel contesto di incontri in grandi e piccoli gruppi, preparati specificamente per rovesciare il corso di quella che era diventata una “malattia del silencio,” e anche per dare ai membri la possibilità di parlare tra loro. Le consultazioni fatte con professionisti esterni all’istituto si sono rivelate utili in questo processo assai arduo dal punto di vista emotivo. Gli autori presentano una serie di ipotesi sulla natura delle ansie di gruppo che emergono quando sono in corso delle crisi che coinvolgono aspetti etici. Essi sostengono che le violazioni dei confini, siano esse di natura sessuale o meno, infrangono comunque il tabù dell’incesto, dal momento che fanno venir meno quella protezione generazionale che è requisito fondamentale delle interazioni professionali. Le violazioni dell’etica attaccano quell’ethos della cura che è fondamento stesso del gruppo, scatenando angosce primitive e difese che vanno a interferire con la capacità di pensare, contenere, collaborare e integrare. Dal momento che è impossibile raggiungere una versione completamente esaustiva di ciò che è accaduto, emergono verità ibride che causano conflitti e disturbi inibendo un discorso di gruppo fondato sul pensiero.
