The exploration of maternal representations during a parenthood-centred psychotherapy from pregnancy to one year postpartum

ABSTRACT

Maternal representations play a key role in intrapsychic conflicts relating to accession to parenthood and in the formation of the mother–baby bond. Around the birth of the child, the shadows of past objects are cast on the baby and the parent's self-image. Mother–baby psychoanalytic psychotherapy helps us to understand internal conflicts that tend to interfere with the mother's representations of her child or of herself as a mother, as well as aiming to reduce the risk of difficulties for the child. Through a clinical case, this article explores the development of maternal representations in a course of parenthood-centred psychotherapy that begins during pregnancy and ends 11 months after the baby's birth. The psychic change carried out by the mother frees the mother–baby bond from the projections of the past. Excerpts from clinical sessions illustrate mother–baby interactions during the session, the projection of the mother's internal objects on to the baby, and the elaboration and reintrojection of the mother's internal conflicts. Changes in the mother's representations were measured both qualitatively and quantitatively using the ‘R’ interview, which allows various dimensions of these representations to be measured on a Likert scale.

Exploration des représentations maternelles au cours d'une psychothérapie centrée sur la parentalité de la grossesse à un an postpartum

Les représentations maternelles tiennent une place centrale dans les conflits intra-psychiques liés à l'accès à la parentalité et la construction du lien mère-bébé. Autour de la naissance d'un enfant, l'ombre des objets du passé se dépose sur le bébé et sur l'image que le parent a de lui-même. Les psychothérapies psychanalytiques mère-bébé aident à comprendre les conflits internes du parent qui tendent à parasiter les représentations qu'il a de son enfant ou de lui-même en tant que parent et visent à diminuer le risque de troubles chez l'enfant. Cet article explore, à travers un cas clinique, le développement des représentations maternelles au cours d'une psychothérapie centrée sur la parentalité initiée pendant la grossesse et qui a pris fin aux 11 mois du bébé. Le changement psychique opéré par la mère permet de libérer le lien mère-bébé des projections du passé. Des extraits de séances cliniques illustrent les interactions mère-bébé en séance, la projection d'objets internes maternels sur le bébé, l'élaboration et la réintrojection des conflits internes de la mère. Les représentations maternelles ont été évaluées de manière qualitative et quantitative au moyen de l'Entretien « R » qui permet de mesurer différentes dimensions de ces représentations sur une échelle Likert.

Die Erforschung von mütterlichen Repräsentationen während einer elternzentrierten Psychotherapie von der Schwangerschaft bis zu einem Jahr nach der Geburt

Mütterliche Repräsentationen spielen eine Schlüsselrolle bei intrapsychischen Konflikten im Zusammenhang mit dem Eintritt in die Elternschaft und bei der Bildung der Mutter-Kind-Bindung. Rund um die Geburt des Kindes werden die Schatten vergangener Objekte auf das Baby und das Selbstbild der Eltern geworfen. Die psychoanalytische Mutter-Baby-Psychotherapie hilft uns, innere Konflikte zu verstehen, die die Repräsentationen der Mutter von ihrem Kind oder von sich selbst als Mutter beeinträchtigen, und zielt darauf ab, das Risiko von Schwierigkeiten für das Kind zu verringern. Anhand eines klinischen Falles wird in diesem Artikel die Entwicklung der mütterlichen Repräsentationen im Rahmen einer auf die Elternschaft ausgerichteten Psychotherapie untersucht, die während der Schwangerschaft beginnt und 11 Monate nach der Geburt des Kindes endet. Die von der Mutter vollzogene psychische Veränderung befreit die Mutter-Baby-Bindung von den Projektionen der Vergangenheit. Auszüge aus klinischen Sitzungen veranschaulichen die Interaktionen zwischen Mutter und Baby während der Sitzung, die Projektion der inneren Objekte der Mutter auf das Baby sowie die Erforschung und Reintrojektion der inneren Konflikte der Mutter. Die Veränderungen in den Repräsentationen der Mutter wurden sowohl qualitativ als auch quantitativ mit Hilfe des “R”-Interviews gemessen, mit dem verschiedene Dimensionen dieser Repräsentationen auf einer Likert-Skala gemessen werden können.

Un'esplorazione delle rappresentazioni materne nel corso di una psicoterapia incentrata sulla genitorialità, dalla gravidanza fino a un anno dal parto

Le rappresentazioni materne hanno un ruolo centrale nei conflitti intrapsichici legati all'ingresso nella genitorialità e alla costruzione del legame madre-neonato. Quando nasce un bambino, l'ombra degli oggetti del passato si proietta sul bambino e sull'immagine di sé del genitore. Le psicoterapie psicoanalitiche madre-neonato aiutano a comprendere i conflitti interni del genitore che tendono a parassitare le rappresentazioni che questi ha del bambino o di se stesso come genitore, e mirano in tal modo a ridurre il rischio di disturbi nel bambino. Concentrandosi su un particolare caso clinico, il presente articolo esamina il progressivo svilupparsi delle rappresentazioni materne nel corso di una psicoterapia incentrata sulla genitorialità iniziata durante la gravidanza e terminata quando il bambino aveva 11 mesi. Il cambiamento psichico operato man mano dalla madre permette di liberare il legame madre-neonato dalle proiezioni del passato. Saranno qui proposte alcune vignette cliniche per illustrare la natura delle interazioni madre-bambino in seduta, la proiezione degli oggetti interni della madre sul bambino, nonché l'elaborazione e la reintroiezione dei conflitti interni da parte della madre. Le rappresentazioni materne sono state valutate con metodi sia qualitativi sia quantitativi utilizzando l'intervista “R”, che permette di misurare le diverse dimensioni di queste rappresentazioni su una scala Likert.

Exploración de las representaciones maternas en el curso de una psicoterapia centrada en la parentalidad desde el embarazo hasta un año posparto

Las representaciones maternas ocupan un lugar central en los conflictos intrapsíquicos ligados al acceso a la parentalidad y la construcción del vínculo madre-bebé. En torno al nacimiento, la sombra de los objetos del pasado se deposita en el bebé y sobre la imagen que los padres tienen de sí mismos. Las psicoterapias madre-bebé ayudan a entender los conflictos internos de la madre que tienden a parasitar las representaciones que tiene de su hijo o de ella misma como madre, siendo también su objetivo disminuir el riesgo de trastornos en el niño. Este artículo explora, a través de un caso clínico, el desarrollo de las representaciones maternas en el curso de una psicoterapia centrada en la parentalidad iniciada durante el embarazo y finalizada a 11 meses del bebé. El cambio psíquico que realiza la madre permite liberar el vínculo madre-bebé de las proyecciones del pasado. Las viñetas de sesiones clínicas ilustran las interacciones madre-bebé en sesión, la proyección de objetos internos maternos sobre el bebé, la elaboración y reintroyección de los conflictos internos maternos. Las representaciones maternas han sido evaluadas de forma cualitativa y cuantitativa mediante la Entrevista “R” que mide diferentes dimensiones de estas representaciones sobre una escala Likert.
