The maximal bicarbonate secretion into the duodenum in response to stepwise increasing doses of secretin did not change by selective proximal vagotomy of the stomach (SPV). The dose of secretin eliciting half-maximal response was higher in patients with duodenal ulcer than in healthy subjects, but decreased after SPV to a value similar to that of the healthy subjects. The results suggest a lower sensitivity of the bicarbonate-secreting cells of the pancreas in duodenal ulcer patients, and that the sensitivity is increased by SPV. Also for enzyme secretion SPV apparently shifted the dosc-response curve to the left.
