The Clinical Manifestations, Natural History and Results of Treatment of Superior Oblique Myokymia

Abstract

Superior oblique myokymia is a disorder of unknown etiology, that is characterized symptomatically by oscillopsia, vertical or torsional diplopia, or both. Affected patients show uniocular bursts of small-amplitude, high frequency rotary oscillations; large-amplitude, high-frequency intorsional movements; or a combination of these paroxysms. The condition is rarely associated with any intracranial or systemic diseases and tends to run a chronic course, characterized by exacerbations and remissions. Many patients require only reassurance regarding the benign nature of the condition. Those patients for whom ocular symptoms are intolerable can be treated with medication or surgery, with the latter treatment tending to be more successful than the former.
