A slow unfolding – at double speed: reflections on ways of working with parents and their young children within the Tavistock clinic's under fives service

Abstract

This paper describes clinical work carried out as part of the Tavistock Clinic Under Fives Service, which offers brief focused psychoanalytically based interventions to families with young children, as well as longer-term work. It elaborates psychoanalyst Annette Watillon's idea that it is the dramatic way in which children enact their (and their family's) predicament in the consulting room which speeds up the process of change. The author clusters clinical cases into three categories, each cluster representing a different kind of ‘dramatic’ enactment and leading to a different kind of intervention relating to the therapist's role, the structure of the interventions and the ‘ports of entry’ for the work. The categories are defined as: ‘child-led drama’ with the therapist in the role of ‘therapeutic observer’; ‘internal parental drama’ with the therapist in the role of ‘therapeutic consultant’; and ‘external parental drama’ with the therapist in the role of ‘therapeutic modulator’. The author defines these categories, illustrating each category with clinical examples.

Cet article décrit le travail clinique entrepris dans le cadre du service pour les moins de cinq ans à la clinique Tavistock, service qui propose à des familles ayant de jeunes enfants soit des interventions d'inspiration psychanalytique brèves et à visée précise, soit des traitements à plus long terme. Il s'agit d'un prolongement de l'idée de la psychanalyste Annette Watillon selon laquelle c'est la dramatisation avec laquelle l'enfant met en scène, dans le cabinet de consultation, la situation difficile dans laquelle il (et sa famille) se trouvent qui accélère le processus de changement. L'auteur regroupe les cas cliniques en trois catégories; chaque groupement représente une “dramatisation” différente qui nécessite une intervention spécifique concernant le rôle du thérapeute, la structure des interventions et les “portes d'entrée” de ce travail. Ces catégories sont définies ainsi: “dramatisation sous la direction de l'enfant” dans laquelle le thérapeute est “observateur thérapeutique”; “dramatisation interne parentale” dans laquelle le rôle du thérapeute est celui d'un “consultant thérapeutique”; et “dramatisation externe parentale” dans laquelle le thérapeute est “modulateur thérapeutique”. L'auteur donne une définition de chacune de ces catégories et y associe des exemples cliniques pour illustrer son propos.

Riassunto: L'articolo descrive il lavoro clinico svolto all'interno del Servizio 0-5 della Tavistock Clinic, che offre alle famiglie con bambini piccoli trattamenti a orientamento psicoanalitico di tipo breve, parallelamente a trattamenti più a lungo tempo. Nell'articolo si elabora l'idea della psicoanalista Annette Watillon per la quale è proprio la modalità di espressione usata dai bambini per inscenare il loro dramma (e quello della famiglia) nella stanza di consultazione a velocizzare il processo di cambiamento. L'autrice raggruppa i casi clinici in tre categorie, ciascun gruppo rappresenta un tipo diverso di rappresentazione ‘drammatica’ che porta ad un tipo diverso di intervento in relazione al ruolo del terapeuta, alla struttura degli interventi e ai ‘porti d'entrata’ (Stern, 1995) del lavoro. Le categorie sono definite come: ‘dramma guidato dal bambino’ con il terapeuta nel ruolo di ‘osservatore terapeutico’; ‘dramma genitoriale interno’ con il terapeuta nel ruolo di ‘consulente terapeutico’; e il ‘dramma genitoriale esterno’ con il terapeuta nel ruolo di ‘modulatore terapeutico’. L'autrice definisce ciascuna di queste categorie riportando esempi clinici.

Dieser Artikel beschreibt klinische Arbeit, die als Teil des Beratungsdienst für Unter 5-jährige an der Taviststock Klinik ausgeführt wurde, der fokussierte psychoanalytisch fundierte Kurzinterventionen wie auch langfristige Arbeit anbietet. Es baut auf der Idee der Psychoanalytikerin Annette Watillons auf, dass es die dramatische Art und Weise ist, in dem die Kinder ihre und die Lage ihrer Eltern im Behandlungsazimmer darstellen, die den Veränderungsprozess beschleunigt. Die Autorin gruppiert klinische Fälle in drei Kategorien, jede Gruppe repräsentiert eine verschiedene Art des ‘dramatischen’ Ausagierens und führt zu einer anderen Art der Intervention bezüglich der Rolle des Therapeutens, der Struktur der Interventionen und der ‘Eingangspforten’ (Stern 19954) für die Arbeit. Die Kategorien werden folgendermassen definiert: kindgeführtes Drama mit dem Therapeuten in der Rolle des ‘therapeutischen Beobachters; das innere elterliche Drama mit dem Therapeuten in der Rolle des ‘therapeutischen Beraters’; und das'äussere elterliche Drama mit dem Therapeuten in der Rolle des ‘therapeutischen Modulators’. Die Autorin definiert diese Kategorien, und illustriet jede Kategorie mit klinischen Beispielen.
