Clinical research: a psychotherapeutic assessment model for siblings in care

Abstract

This paper focuses on the aspects of a qualitative research project that examines an assessment protocol for the placement of siblings in foster care and/or future adoption. A brief description of the background to the study and the research design is given. Evaluating the material from the quantitative instruments used and the psychotherapeutic assessments, the author identified two predominant themes. The first, cumulative trauma, pointed to the pervasive nature of the children's early experiences, as manifest through their presentation and play. The second, relatedness and belonging, highlighted aspects of the children's shared memories and experiences. Clinical vignettes are used to illustrate the way in which these themes related to the children's sibling relationships. The author reflects on the methodology used that allowed the complexity of the issues being studied to ‘come to life’. The study noted the tendency for those caring for or working with the children to underestimate the children's meaning to each other and emphasised the importance of assessing the children's perspectives where decisions are being made in childcare cases. Further questions were raised as to whether more could be done to mediate or facilitate sibling relationships for children who have suffered early deprivation or abuse, and the need for further research into this area of work was proposed.

Cet article décrit certains aspects d'un projet de recherche qualitative portant sur un protocole d'évaluation en vue du placement en famille d'accueil et / ou éventuellement adoptive des membres d'une même fratrie. L'auteur fournit une brève description du contexte dans lequel cette étude fut entreprise et du protocole de recherche. La prise en compte du matériel recueilli grâce aux instruments quantitatifs employés et aux évaluations psychothérapeutiques permet le repérage de deux thématiques prédominantes. La première – l'accumulation de traumatismes multiples – souligne la nature omniprésente du vécu précoce des enfants, tel qu'il se manifeste à travers leur conduite et leurs jeux. La seconde – liens de famille et sentiment d'appartenance – met en lumière certains aspects des souvenirs et vécus partagés par ces enfants. Des exemples cliniques illustrent les rapports entre ces thématiques et les relations fraternelles qu'entretiennent ces enfants. L'auteur livre ses réflexions à propos de la méthodologie employée, grâce à laquelle la complexité des questions soulevées a pu être rendue aussi “ vivante ”. Cette étude relève la tendance, chez ceux qui prennent en charge ces enfants, à sous-estimer la signification qu'a chaque enfant pour ses frères et sœurs et souligne l'importance de la prise en compte du point de vue des enfants lorsqu'il s'agit de décider d'un placement éventuel. L'auteur s'interroge sur la possibilité de promouvoir ou de faciliter davantage les relations fraternelles chez les enfants ayant souffert de carences affectives ou de maltraitance précoces et souligne la nécessité d'entreprendre d'autres recherches dans ce domaine.

Questo articolo si focalizza sugli aspetti di un progetto di ricerca qualitativa che esamina un protocollo di valutazione per la sistemazione dei fratelli in affidamento e/o per adozione futura. Viene fatta una breve descrizione dello studio e del progetto di ricerca. L'autrice identifica due temi predominanti che emergono dall'analisi degli strumenti quantitativi utilizzati e dalle valutazioni psicoterapeutiche. Il primo, il trauma cumulativo, fa luce sulla natura pervasiva delle prime esperienze dei bambini come si manifestano nella loro presentazione e nel gioco. Il secondo, capacità di mettersi in relazione e appartenenza, mettono in risalto aspetti delle memorie ed esperienze condivise dei bambini. Si portano degli esempi clinici per illustrare come questi temi si rapportano alle relazioni con i fratelli dei bambini. L'autrice riflette sulla metodologia utilizzata che ha permesso di ‘portare alla luce’ la complessità delle questioni in esame. La ricerca mette in risalto la tendenza in coloro che si prendono cura e lavorano con i bambini a sottovalutare l'importanza che i bambini hanno uno per l'altro ed enfatizza l'importanza di valutare le prospettive dei bambini quando si prendono decisioni relative ai casi in affidamento. Vengono sollevate ulteriori questioni relative al fatto se si possa fare di pi[ugrave] per mediare e facilitare le relazioni tra fratelli per i bambini che hanno sofferto deprivazioni precoci o abusi. Si sottolinea il bisogno di fare ulteriori ricerche in quest'area di lavoro.

Dieser Aritel konzentriert sich auf die Aspekte eines qualitativen Forschungsprojekt, , das ein Diagnoseprotokoll für die Plazierung von Geschwistern in Pflege und/oder spätere Adoption betrachtet. Es wird eine kurze Beschreibung des Hintergrunds dieser Studie und des Forschungsdesigns gegeben. Die Autorin identifizierte zwei vorherrschende Themen, als sie das Material von den benutzten quantitativen Instrumenten und den psychotheerapeutischen Erstgesprächen evaluierte. Das erste Thema, kumulatives Trauma, wies auf die vorherrschende Art der frühen Erlebnisse der Kinder wie es in ihren Präsentierung und Spiel deutlich wurde, hin. Das zweite Thema, Bezogenheit und Zugehörigkeit, stellt Aspekte der von den Kindern geteilten Erinnerungen und Erlebnisse heraus. Es werden klinische Vignetten benutzt, um aufzuzeigen, wie sich diese Themen auf die Geschwisterbeziehungen bezogen. Die Autorin reflektiert die angewandte Methodologie, die es ermöglichte, die Komplxität der studierten Themen ‘zum Leben zu bringen’. Die Studie deckte eine Tendenz bei denen auf , die Pflegeeltern sind oder mit den Kindern arbeiten, die Bedeutung der Kinder füreinander zu unterschätzen , und sie betonte die Wichtigkeit, die Perspektive der Kinder einzuschätzen, wenn Entscheidungen in Sorgerechtsfällen getroffen werden. Es werden weitere Fragen aufgeworfen, ob mehr getan werden könnte, um Geschwisterbeziehungen zu meditieren oder zu erleichtern für Kinder, die frühe Deprivation oder Missbrauch erlitten hatten und es wird der Bedarf an weiterer Forschung in diesem Bereich festgestellt.
