An error was reported in the response rate for the sample obtained via the Surrey cohort (table 1). Sixteen questionnaires not 31 were sent out, resulting in a useable response rate of 62.5% not 32% as reported. This means a total of 110 (rather than 125) questionnaires were sent out in total and the overall usable response rate was 71% rather than 63%. In addition, all the aforementioned sixteen questionnaires were sent to individuals who met the inclusion criteria, therefore reproductive age is unlikely to have been a reason for non‐responding as suggested. The authors would like to apologise to Dr Janet Carr and thank her for identifying the error.
