Abstract
In 2006, the United Hospital Fund and New York City's Department for the Aging began a joint undertaking in 34 naturally occurring retirement community (NORC) programs to reduce risks to healthy aging. The Health Indicators in NORC Programs Initiative is a data-driven, risk-identification tool and a quality-improvement process designed to help community-based programs with limited resources identify key health risks among older adults in their communities, develop targeted and systematic responses, and measure the effectiveness of those responses. This article presents findings from three NORC programs to illustrate the importance of understanding each community's risks to healthy aging in developing effective targeted responses.
