Notes
1. The authors thank Jan Greyling for his review of a prior draft of these reflections on our mutual friend Frikkie.
2. As Hulten (Citation2001) wrote “Griliches (Citation1996) credits the first mention of the output per unit input index to Copeland (Citation1937), followed by Copeland and Martin (Citation1938). The first empirical implementation of the output per unit input index is attributed to Stigler (Citation1947)”.
3. The InSTePP US Production Accounts are available at www.instepp.umn.edu/united-states.
4. The authors acknowledge that the success of their efforts lay largely in the hands of Niels Meyer at the SA Department of Agriculture whose extensive statistical knowledge was indispensable in tracking down important but obscure, and sometimes unpublished, data sources. This issue also serves as a tribute to Niels. For a recasting and revision of the output series reported by Thirtle et al. (Citation1993), see Liebenberg et al. (Citation2015).
5. Everybody that worked with Frikkie in those years would remember his detailed data work, his large spreadsheets, his macros, and his excellent working knowledge of Excel. Many also knew of his long working hours, almost always starting his day at three in the morning.
