In this manuscript, two Data to Care models implemented in the same geographic location were directly compared. While it is stated in the paper that only cases from January 1, 2015 - September 1, 2016 were included in the analysis for both models, the Health Department Model included cases from September 1, 2013 - September 1, 2016.

Please note that we have re-run the analysis restricting the cases for the Health Department Model to mirror that of the Combination Model, and the overall results and conclusion remain unchanged with the exception of the overall sample size.
