Patients’ experiences of change in cognitive–behavioral therapy and psychodynamic therapy: a qualitative comparative study

Abstract

Research suggests that different therapeutic approaches produce roughly equivalent outcomes despite their theoretical and technical differences. This study explores whether the quantitative equivalence may conceal differences in patients’ experiences of the quality of change. Thirty-two patients who had terminated cognitive–behavioral therapy or psychodynamic therapy were interviewed about their experiences in psychotherapy. The cases were clustered in four groups depending on type of therapy and whether outcome was judged as satisfactory or not. An outcome narrative was reconstructed for each type. Even though the ratio of satisfactory to unsatisfactory cases was roughly equal in the two types of treatments, there were obvious differences in the kinds or quality of outcome that were reported as well as some experiences common to the two groups of patients. The narratives of the dissatisfied patients underscored the importance of matching the approach to the patient.

Veränderungs-Erfahrung der Patienten in kognitiver Verhaltenstherapie und psychodynamischer Therapie: eine qualitative Vergleichsstudie

Forschungsergebnisse legen nahe, dass unterschiedliche therapeutische Richtungen zu fast gleichem Therapie-Outcome führen, trotz theoretisch und technischer Unterschieden. Diese Studie untersucht, ob die quantitative Äquivalenz Unterschiede in der Veränderungs-Erfahrung der Patienten verdeckt. Es wurden 32 Patienten bezüglich ihrer Erfahrungen in der Psychotherapie (entweder kognitive Verhaltenstherapie oder psychodynamische Therapie) interviewt. Die Fälle wurden in vier Gruppen geclustert, abhängig von der Art der Therapie und ob die Therapie als zufrieden stellend bzw. nicht-zufriedenstellend bewertet wurde. Ein Outcome-Bericht wurde für jedes Cluster rekonstruiert. Obwohl die Anzahl zufriedenstellender und nicht-zufriedenstellender Therapien in beiden Therapierichtungen annähernd gleich war, gab es offensichtliche Unterscheide in der Art und Weise der Therapie und der Qualität des Outcomes, ebenso einige gleiche Erfahrungen in den beiden Patientengruppen. Die Berichte der unzufriedenen Patienten unterstreichen die Wichtigkeit der Passung zwischen therapeutischem Ansatz und Patient.

L'expérience de changement du patient en thérapie cognitive comportementale ou psychodynamique: une étude qualitative comparative

La recherché suggère que des méthodes différentes produisent des résultats largement équivalents malgré leurs différences théoriques et techniques. Cette étude explore la possibilité que l’équivalence quantitative pourrait cacher des différences dans les expériences du patient de la qualité du changement. 32 patients ayant terminé une thérapie soit cognitive comportementale, soit psychodynamique étaient interviewés sur leurs expériences en psychothérapie. Les cas étaient répartis en clusters de 4 groupes en fonction du type de thérapie et d'un résultat jugé comme satisfaisant ou pas. Un narratif de résultat était reconstruit pour chaque type. Même si la proportion de cas satisfaisants versus insatisfaisants était à peu près égale pour les deux types, il y avait des différences évidentes dans le type et la qualité du résultat rapporté ainsi que quelques expériences communes dans les deux groupes de patients. Les narratifs des patients insatisfaits soulignent l'importance de l'accordement de l'approche du patient.

Experiencias de cambio en pacientes en terapia cognitivo-comportamental y terapia psicodinámica: estudio cualitativo comparativo

La investigación sugiere que enfoques terapéuticos diferentes producen aproximadamente resultados equivalentes no obstante sus diferencias teóricas y técnicas. Este estudio explora la posibilidad de que la equivalencia cuantitativa pueda encubrir diferencias en las experiencias de los pacientes respecto de la calidad del cambio. Se entrevistó a treinta y dos pacientes que habían terminado su terapia cognitivo-comportamental o su terapia psicodinámica acerca de sus experiencias en psicoterapia. Los casos fueron agrupados en cuatro grupos según el tipo de terapia y según que el resultado fuera considerado satisfactorio o no. Se reconstruyó una narrativa del resultado en cada tipo. Aun cuando la relación entre casos satisfactorios y no satisfactorios fue aproximadamente igual en los dos tipos de tratamientos, hubo diferencias obvias en la clase de calidad del resultado informado así también como algunas experiencias communes en ambos grupos de pacientes. Las narrativas de los pacientes insatisfechos subrayó la importancia de empardar el enfoque del paciente.

A investigação sugere que diferentes abordagens terapêuticas produzem resultados terapêuticos de algum modo semelhantes apesar das diferenças orientaç[otilde]es teóricas e técnicas. Este estudo explora se uma equivalência quantitativa pode ocultar diferenças nas experiências dos pacientes na qualidade da mudança. Vinte e dois pacientes que haviam terminado terapia cognitivo-comportamental ou terapia psicodinâmica foram entrevistados sobre as suas experiências em psicoterapia. Os casos foram divididos em 4 grupos dependendo do tipo de terapia e se os resultados foram considerados satisfatórios ou não. Foi construída uma narrativa para os resultados para cada tipo. Embora a proporção de casos satisfatórios e insatisfatórios tenha sido semelhante nos dois tipos de tratamento, existiram diferenças óbvias em relação aos tipos ou qualidade dos resultados que foram referidos, assim como em relação a algumas experiências comuns para os dois grupos de pacientes. As narrativas dos pacientes insatisfeitos mostraram a importância de fazer adequar as abordagens ao paciente.

Le esperienze di cambiamento dei pazienti in terapia cognitivo-comportamentale e in terapia psicodinamica: uno studio comparativo qualitativo

La ricerca suggerisce che differenti approcci terapeutici producono esiti equivalenti, malgrado le differenze le differenze teoriche e tecniche. Questo studio esplora se l'equivalenza quantitativa nasconda differenze nelle esperienze dei pazienti della qualità del cambiamento. Abbiamo intervistato 32 pazienti che avevano concluso una terapia cognitivo-comportamentale o psicodinamica sulla loro esperienza della psicoterapia. I casi sono stati raggruppati in 4 gruppi in base al tipo di terapia e alla soddisfazione verso il trattamento. Abbiamo ricostruito una narrativa di esito per ciascun tipo. Sebbene il rapporto tra casi soddisfatti e insoddisfatti fosse eguale nei due tipi di trattamento, c'erano ovvie differenze nel tipo o nella qualità dell'esito riportato. Ma c'erano anche esperienze comuni. Le narrative dei pazienti insoddisfatti sottolineavano l'importanza del buon accoppiamento tra paziente e approccio terapeutico.

1. We use the term “quasi-qualitative” for studies in which more or less predetermined coding categories or rating scales are used to analyze free-format responses to more or less fixed questions. Results are typically given in terms of frequency distributions across the categories (see, e.g., Gershefski et al., Citation1996). This is in contrast to qualitative studies, in which informal or conversational interviews are processed so as to construct or reconstruct and summarize more or less interpretative narratives.

3. It deserves to be emphasized that a “satisfactory case” is not simply one in which the patient says she feels good about the therapy, that she and the therapist had a good relation, and so on. In fact, there were cases in which the patient declared herself being quite satisfied that were classified as unsatisfactory and vice versa.

4. The patient who appeared psychotic during the interview was excluded in this calculation because her confused and unrealistic account made it impossible to classify the case as either satisfactory or unsatisfactory.
