Mental health characteristics of patients assigned to long-term ambulatory psychoanalytic psychotherapy and psychoanalysis in the netherlands

Abstract

The present study investigated mental health characteristics of 170 patients assigned to long-term psychoanalytic psychotherapy (PP) versus psychoanalysis (PA) across treatment and studies. Both univariate and multivariate statistics showed that the main difference between treatments was found in the interpersonal domain. PA patients reported significantly more interpersonal problems (as measured by the Inventory for Interpersonal Problems-64), scored higher on the avoidant coping style and lower on the perceptual thinking index of the Rorschach-Comprehensive System, and scored lower on Aggressiveness and Psychoticism Minnesota Multiphasic Personality Inventory-2 PSY-5 scales compared with PP patients. Compared with psychoanalytic patients in other studies, our patients had similar levels of mental health problems, although not always in the same health areas. Limitations of the study were noted, such as the lack of outcome data and other comparison groups.

Die vorliegende Studie untersuchte Merkmale psychischer Störungen von 170 Patienten, die entweder an einer psychoanalytischen Langzeitpsychotherapie (PP) oder an einer Psychoanalyse (PA) teilnahmen, über Behandlungen und Studien hinweg. Sowohl die univariate wie auch die multivariate Statistik zeigte, dass der Hauptunterschied zwischen Behandlungen im interpersonalen Bereich lag. Patienten, die an der PA teilnahmen, berichteten signifikant mehr Probleme im interpersonalen Bereich (gemessen mit dem Inventory for Interpersonal Problems-64), erzielten höhere Werte im vermeidenden Coping, niedrigere im Wahrnehmungsindex des Rohrschach-Tests und auf den Skalen Aggressivität und Psychotizimus des Minnesota Multiphasic Personality Inventory-2, als die Patienten, die an der PP teilnahmen. Verglichen mit psychoanalytisch-behandelten Patienten in anderen Studien, hatten diese Patienten ähnliche Levels psychischer Probleme, allerdings nicht immer in den gleichen Bereichen. Einschränkungen der Studie wurden berichtet, sowie das Fehlen von Outcome-Daten und anderen Vergleichsgruppen.

L’étude investigue les caractéristiques de santé mentale de 170 patients attribués à une psychothérapie psychanalytique à long terme (PP) ou à une psychanalyse (PA). Les statistiques univariées et multivariées montrent toutes les deux que la différence principale entre les traitements concernaient le domaine interpersonnel. Les patients du groupe PA signalaient significativement plus de problèmes interpersonnels (mesurés par l'Inventaire de Problèmes Interpersonnels - 64), scoraient plus haut sur le style de coping évitant, scoraient plus bas sur l'index de pensée perceptuelle du système du Rorschach en système intégré, et scoraient plus bas sur les échelles d'agressivité et de psychotisme de l'Inventaire Multiphasique de Personality du Minnesota – 2 en comparaison des patients du groupe PP. Comparés avec les patients psychanalytiques d'autres études, nos patients avaient des niveaux de problèmes de santé mental similaires, bien que pas toujours dans les mêmes secteurs de la santé. Des limitations de l’étude sont évoquées, comme l'absence de données de résultats et d'autres groupes de comparaison.

Este estudio investigó las características de la salud mental de ciento setenta pacientes asignados a psicoterpia psicoanalítica a largo plazo (PP) versus psicoanálisis (PA) a lo largo del tratamiento y otros estudios. Tanto las estadísticas univariadas como las multivariadas mostraron que la principal diferencia entre los tratamientos radicaba en el dominio interpersonal. Los pacientes de PA reportaron significativamente más problemas interpersonales (según medición por el Inventario de Problemas Interpersonales-64), puntuaron más alto en el estilo evitativo de afrontamiento, más bajo en el índice de pensamiento perceptual del Sistema Rorschach-Comprehensivo y más bajo en las escalas de Agresividad y Psicotismo del Inventario-2 Psy-5 comparados con los pacientes de PP. Comparados con los pacientes psicoanalíticos en otros estudios, los nuestros tuvieron niveles similares de problemas de salud mental, si bien no siempre en las mismas áreas de salud. Se señalaron las limitaciones del estudio, tales como la falta de datos de resultados y otros grupos de comparación.

Questo studio ha investigato le caratteristiche di salute mentale di 170 pazienti assegnati a psicoterapia psicoanalitica a lungo termine (PP) contro psicoanalisi (PA). Sia la statistica multivariata che univariata hanno mostrato che le maggior differenze risiedevano nell'ambito interpersonale. Rispetto ai pazienti PP, i pazienti PA riferivano pi[ugrave] problemi interpersonali (misurati con l'Inventory for Interpersonal Problems-64), avevano punteggi pi[ugrave] alti nello stile di fronteggiamento evitante e pi[ugrave] bassi nell'indice di pensiero percettivo del Rorschach-Comprehensive System e nelle scale Aggressività e Psicoticismo del Minnesota Multiphasic Personality Inventory-2 PSY-5. Rispetto a pazienti in psicoanalisi di altri studi, i nostri pazienti avevano livelli simili di problemi di salute mentale, sebbene non sempre nelle stesse aree. Sono stati notati alcuni limiti di questi studi, come la mancanza di dati di esito e di gruppi di controllo.

1. A third instrument was the Adult Attachment Interview developed by Main and Goldwyn (Citation1998). Although the instrument is part of the routine outcome monitoring, these interviews have not been scored because of the costs of transcription and coding.
