Mental states as part of countertransference responses in psychotherapists facing reports of traumatic events of mourning and sexual violence

Abstract

The study aims to compare the mental states and countertransference responses of 92 psychodynamically oriented psychotherapists, male and female, experienced and inexperienced, facing written reports of real patients who experienced traumatic events. Two vignettes were presented: one of a sexual violence, the other the sudden death of a significant person. The Mental States Rating System (MSRS; Bouchard, Picard, Audet, Brisson, & Carrier, 1998), the MSRS Self-Report (Goldfeld & Bouchard, 2004), and the Inventory of Countertransference Behavior (ICB; Friedman & Gelso, 2000) were used. Results showed that the mourning vignette led to more reflective responses (MSRS) and the rape case was associated with more negative countertransference reactions (ICB). Female participants were more reflective (MSRS); male therapists used less mentalized states (MSRS Self-Report) and expressed more negative reactions (ICB) for both scenarios. Experienced therapists showed more positive reactions on the ICB. The construct validity of the instruments is discussed in relation to the findings.

Die Studie vergleicht die geistigen Zustände und gegenübertragenden Antworten 92 psychodynamisch-orientierter Psychotherapeuten – männlich und weiblich, erfahren und unerfahren – die sich geschriebene Berichte über traumatische Erlebnisse realer Patienten ansahen. Zwei Vignetten wurden präsentiert: eine über sexuelle Gewalttat, die andere über den plötzlichen Tod einer wichtigen Person. Das Mental States Rating System (MSRS; Bouchard, Picard, Audet, Brisson, & Carrier, 1998), und der MSRS Self-Report (Goldfeld & Bouchard, 2004), sowie das Inventory of Countertransference Behavior (ICB; Gelso & Friedman, 2000) wurden verwendet. Die Ergebnisse zeigten, dass die Trauer-Vignette zu mehr reflektierenden Antworten führte (MSRS) und der Vergewaltigungsfall war mit mehr negativen gegenübertragenden Reaktionen (ICB) assoziiert. Weibliche Teilnehmer waren reflektierender (MSRS); männliche Therapeuten gebrauchten weniger geistige Zustände (MSRS Self-Report) und drückten in beiden Szenarien mehr negative Reaktionen (ICB) aus. Erfahrene Therapeuten zeigten mehr positive Reaktionen im ICB. Die Konstruktvalidität der Instrumente wird im Zusammenhang mit den Ergebnissen diskutiert.

L’étude cherche à comparer les états mentaux et la réponse contre-transférentielle de 92 psychothérapeutes d'orientation psychodynamique, hommes et femmes, avec ou sans expérience, qui font face à un rapport écrit de cas de patients réels qui ont expérimenté des événements traumatiques. Deux vignettes ont été présentées : une de violence sexuelle, l'autre de la mort subite d'une personne significative. Le système de cotation des états mentaux (MSRS; Bouchard, Picard, Audet, Brisson, & Carrier, 1998), le MSRS-Autorapporté (Goldfeld & Bouchard, 2004), and l'inventaire des comportements de contre-transfert (ICB; Gelso & Friedman, 2000) ont été utilisés. Les résultats ont montré que la vignette de deuil conduit à une réponse plus réflexive (MSRS) et que le cas de viol était associé avec une réaction contre-transférentielle plus négative (ICB). Les femmes étaient plus réflexives (MSRS) ; les thérapeutes homme utilisaient moins d’états mentalisés (MRSR-Autorapporté) et exprimaient plus de réactions négatives (ICB) pour les deux scénarios. Les thérapeutes expérimentés montraient plus de réactions positives à l'ICB. La validité de construction des instruments est discutée en relation avec ces résultats.

El estudio tiende a comparar los estados mentales y las respuestas contratransferenciales de noventa y dos psicoterapeutas de orientación psicodinámica, varones y mujeres, experimentados e inexperimentados, ante informes escritos de pacientes reales que experimentaron hechos traumáticos. Se presentaron dos viñetas: una de violencia sexual, la otra de muerte súbita de una persona significativa. Se usaron el Sistema de Puntaje de Estados Mentales (MSRS; Bouchard, Picard, Audet, Prisson & Carrier, 1998), el Informe Personal MSRS (MSRS-Self Report (Goldfeld & Bouchard, 2004) y el Inventario de Comportamiento Contratransferencial (IIB; Gelso & Friedman, 2000). Los resultados mostraron que la viñeta de duelo arrojó respuestas más reflexivas (MSRS) y que el caso de violación se asoció con reacciones contratransferenciales más negativas ( ICB). Las participantes femeninas fueron más reflexivas (MSRS); los terapeutas varones usaron menos estados (MSRS-Informe Personal) y expresaron reacciones más negativas (ICB) para ambos escenarios. Los terapeutas experimentados mostraron reacciones más positivas a los ICB. Se debate sobre la validez de los instrumentos en relación con los hallazzgos.

Abstract

O objectivo deste estudo é comparar estados mentais e respostas de contra transferência de 92 psicoterapeutas de orientação psicanalítica do sexo masculino e feminino, experientes e inexperientes, em relação a descrições escritas de pacientes reais que viveram eventos traumáticos. Dois tipos de casos foram apresentados: um de violência sexual e outro de morte súbita de uma pessoa significativa. Foram usados o Mental States Rating System (MSRS; Bouchard, Picard, Audet, Brisson, & Carrier, 1998), the MSRS Self-Report (Goldfeld & Bouchard, 2004) e o Inventory of Countertransference Behavior (ICB; Gelso & Friedman, 2000). Os resultados mostram que a os casos de luto levavam a respostas mais reflexivas (MSRS); os terapeutas do sexo masculino utilizavam menos estados de mentalização (MSRS – Auto-Relato) e expressavam mais emoções negativas (ICB) para ambos os cenários. Os terapeutas experientes mostraram mais reacções positivas no ICB. A validade de constructo dos instrumentos é discutida tendo em conta os resultados obtidos.

Lo studio confronta gli stati mentali e le risposte controtransferali di 92 terapeuti orientati psicodinamicamente, maschi e femmine, esperti e inesperti, che trattano rapporti scritti di pazienti reali che hanno subito eventi traumatici. Sono state presentate due vignette: una di violenza sessuale, l'altra di morte improvvisa di persone significative. Sono stati usati il Mental States Rating System (MSRS; Bouchard, Picard, Audet, Brisson, & Carrier, 1998), il MSRS Self-Report (Goldfeld & Bouchard, 2004) e il Inventory of Countertransference Behavior (ICB; Gelso & Friedman, 2000). I risultati hanno mostrato che la vignetta di lutto portava a risposte più riflessive (MSRS) e il caso di stupro era associato a risposte controtransferali più negative (ICB). I partecipanti donne erano più riflessive (MSRS); i terapeuti maschi usavano meno gli stati mentalizzati (MSRS Self-Report) ed esprimevano più reazioni negative (ICB) per entrambi gli scenari. I terapeuti esperti mostravano più reazioni positive sull'ICB. Si discute la validità del costrutto dello strumento in relazione ai risultati.
