Abstract
The present study is the first aimed at investigating the influence of goal definition on treatment outcome in a sample of depressed patients. Data from 657 inpatients admitted to a psychosomatic clinic in Germany being treated in a cognitive–behavioral therapy program were analyzed. Treatment goals were identified as either approach or avoidance, and the sample was classified accordingly. Patients who identified approach goals only were placed in the approach group, and those who identified at least one avoidance goal were placed in the avoidance group. Results showed that framing goals using avoidance terms was associated with less symptomatic improvement but did not affect goal attainment. Findings from this research should be utilized in practice not only for process management such as individual treatment planning but also to control outcome quality. Furthermore, goal definition should be considered as a control variable in research on depression.

Cette étude est la première à investiguer l'influence de la définition des but sur les résultats du traitement dans un échantillon de patients déprimés. Des données de 657 patients hospitalisés admis dans une clinique psychosomatique en Allemagne traités avec un programme de thérapie cognitive-comportementale ont été analysées. Les buts du traitement ont été identifiés comme soit de l'approche, soit de l’évitement, et l’échantillon classé suivant cette catégorisation. Les patients qui ont identifié seulement des buts d'approche ont été placés dans le groupe approche et les patients qui ont identifié au moins un but d’évitement ont été placés dans le groupe évitement. Les résultats ont montré que les buts utilisant des termes d’évitement étaient associés avec moins d'amélioration symptomatique mais n'affectaient pas l'atteinte des buts. Ces résultats peuvent être utilisés dans la pratique non seulement comme gestion du processus telle que la planification individuelle du traitement mais également comme contrôle de la qualité des résultats. De plus, la définition de buts peut être considérée comme une variable contrôle dans la recherche sur la dépression.
Abstract
Il presente studio indaga per la prima volta gli obiettivi e il risultato del trattamento in un campione di pazienti depressi. Sono stati analizzati i dati di 657 pazienti ricoverati in una clinica in Germania trattati con un approccio psicosomatico in un programma di terapia cognitivo-comportamentale. Gli obiettivi della terapia sono stati identificati in base sia alla teoria sia agli scopi e il campione è stato classificato di conseguenza. I pazienti che hanno individuato come unico approccio gli obiettivi sono stati collocati nel gruppo di approccio, quelli che hanno identificato almeno un obiettivo di evitamento sono stati collocati nel gruppo di evitamento. I risultati hanno mostrato che coloro che utilizzano l'evitamento come strategia presentano un miglioramento meno sintomatico, che non gli ha impedito di raggiungere l'obiettivo prefissato. I risultati di questa ricerca dovrebbe essere utilizzati, in pratica, non solo per la gestione dei processi come la pianificazione del trattamento individuale, ma anche al controllo di qualità dell'esito. Inoltre, la definizione degli obiettivi deve essere considerata come una variabile di controllo in materia di ricerca sulla depressione
Abstract
Die vorliegende Studie untersuchte als erste den Einfluss von Zielsetzungen auf das Therapieergebnis an einer Stichprobe depressiver Patienten. Daten von 657 stationären Patienten, welche in einer psychosomatischen Klinik in Deutschland mit einem kognitiven verhaltenstherapeutischen Programm behandelt wurden, wurden analysiert. Behandlungsziele wurden entweder als Annäherung oder als Vermeidung identifiziert und die Stichprobe wurde dementsprechend klassifiziert. Patienten, welche ausschließlich Annäherungsziele festlegten, wurden in die Annäherungsgruppe, Patienten, die mindestens ein Vermeidungsziel festlegten in die Vermeidungsgruppe platziert. Die Ergebnisse zeigten, dass die Aufteilung von Zielen unter Verwendung der Bezeichnung Vermeidung im Zusammenhang mit weniger symptomatischer Verbesserung stand, allerdings nicht die Zielerreichung beeinflusste. Diese Forschungsergebnisse sollten in der Praxis nicht nur für das Prozessmanagement wie zum Beispiel für die individuelle Behandlungsplanung angewendet, sondern auch um die Qualität der Therapieergebnisse zu kontrollieren, berücksichtigt werden. Darüber hinaus sollte die Zieldefinition als eine Kontrollvariable in der Depressionsforschung berücksichtigt werden.
