Telehealth approaches to obsessive-compulsive related disorders

Abstract

Recently, a major new category classification of disorders, the obsessive-compulsive related disorders (OCRDs), has been proposed (Hollander, Braun, & Simeon, 2008). The proposed disorders that would fall in this category are associated with efficacious treatments, but have not been widely disseminated. Recent developments in telehealth have made treatment available to a wider range of clients with putative OCRDs. This paper evaluates the efficacy of these treatments when delivered using telehealth methodologies. While the evidence is preliminary, the early results are encouraging for the delivery of cognitive-behavior therapy for a wide range of OCRDs. Additional research directions are suggested in light of these emerging technologies for treatment delivery.

Recentemente è stata proposta una nuova macro categorizzazione, i disturbi ossessivo-compulsivi correlati (OCRDs) (Hollander, Braun, & Simeon, 2008). I disturbi proposti che dovrebbero ricadere in questa categoria sono associati a trattamenti efficaci, che non sono tuttavia ancora ampiamente utilizzati. Il recente sviluppo di percorsi di “tele salute” ha reso molti trattamenti disponibili per un più ampio range di clienti con presunti OCDRs. Il presente articolo valuta l'efficacia di questi trattamenti proposti utilizzando queste metodologie. Sebbene i dati siano da considerarsi preliminari, i primi risultati sembrano incoraggiare le proposte per terapie cognitivo-comportamentali per un ampio range di OCRD. Ulteriori prospettive di ricerca verranno proposte alla luce di queste nuove tecnologie emergenti per le modalità di presentazione del trattamento.

Récemment, une nouvelle catégorie majeure de classification de troubles, les troubles reliés aux troubles obsessionnels-compulsifs (OCRDs) a été proposée (Hollander, Braun, & Simeon, 2008). Les troubles proposés qui tombent dans cette catégorie sont associés avec des traitements efficaces, mais qui n'ont pas encore été largement disséminés. Les développements récents dans la télésanté ont rendu le traitement disponible pour une gamme de patients plus large avec un OCRD putatif. Cet article évalue l'efficacité de ces traitements lorsqu'ils sont délivrés en utilisant les méthodologies de la télésanté. Bien que la preuve ne soit que préliminaire, ces premiers résultats sont encourageants pour fournir une thérapie cognitive-comportementale à un large éventail d'OCRDs. Des directions de recherches additionnelles sont suggérées à la lumière de ces technologies émergentes pour la fourniture d'un traitement.

As perturbações propostas que se enquadram nesta categoria e estão associadas a tratamentos eficazes, no entanto não foram amplamente divulgadas. Desenvolvimentos recentes em telehealth tornaram o tratamento disponível para um elevado número de pacientes com supostas OCRDs. Este artigo avalia a eficácia destes tratamentos na utilização de metodologias de telehealth. O estudo preliminar evidencia que os primeiros resultados são encorajadores na utilização de terapia cognitivo-comportamental para um elevado número de OCRDs. As investigações adicionais sugerem a emergência destas tecnologias no tratamento.

Abstract

Aktuell wurde eine grundlegend neue Kategorie zur Klassifikation von Störungen, die zwangsbezogenen Störungen (obsessive-compulsive related disorders - OCRDs) vorgeschlagen (Hollander, Braun, & Simeon, 2008). Die vorgeschlagenen Störungen, welche unter diese Kategorie fallen würden, können mit wirksamen Behandlungen in Verbindung gebracht werden, welche allerdings noch keine ausreichende Verbreitung gefunden haben. Neuere Entwicklungen im Bereich der Telegesundheit haben Behandlungen für eine Vielzahl von Klienten mit einer mutmaßlichen OCRDs zugänglich gemacht. Der vorliegende Artikel evaluiert die Effektivität solcher Behandlungen, wenn sie über Methoden der Telegesundheit übermittelt werden. Unter dem Vorbehalt, dass die Evidenz zunächst vorläufig ist, befürworten die ersten Ergebnisse den Einsatz kognitiv-behavioraler Behandlungsmethoden für eine Vielzahl zwangsbezogener Störungen. Angesichts der neu entstehenden Technologien zur Behandlungsübermittlung werden zusätzliche Forschungsrichtungen vorgeschlagen.

1. The underlying basis of the OCRD has been the source of controversy, even as it appears imminent for the next edition of the DSM. For an account of the controversies in this diagnostic class, see Abramowitz et al. (2009). As of this writing, the candidate disorders for the OCRDs have been modified and are more restrictive than the range of conditions suggested in Hollander, Simeon, & Braun (2008).

2. At the present time, hoarding is considered part of OCD, but there is a growing consensus that hoarding is substantially different from other symptoms of the diagnosis. This includes onset, pathophysiology, and response to treatment, whether similar to that used for other OCD symptoms or individually tailored for hoarding (Mataix-Cols et al., Citation2010)
