Alliance, reactions to treatment, and counter-transference in the process of recovery from suicidal phenomena in long-term dynamic psychotherapy

Abstract

Objective: We studied therapeutic processes associated with improvement in suicidality in a naturalistic study of long-term dynamic psychotherapy. Methods: In a naturalistic study of long-term dynamic psychotherapy (n = 53), 31 patients reported suicidal ideation, eight of whom also reported prior suicide attempts, while 22 were non-suicidal. Suicide ideation and attempts and components of therapy alliance were followed at 6 month intervals. Results: Suicidal patients had more psychiatric diagnoses, were treated longer and reported more negative reactions to treatment than non-suicidal patients. Alliance moderated (a) the relationship between negative reaction to treatment and improvement in suicidal ideation and (b) the relationship between quality of patient-therapist interaction and improvement. Conclusions: Suicidal patients manifest hostility and negative reactions to treatment, indicating negative transference. Avoiding therapist errors emanating from negative countertransference improves the therapeutic interaction, which is followed by faster rate of improvement in suicidality.

Objetivo: estudámos processos psicoterapêuticos associados a melhoria em fenómenos suicidas num estudo naturalista de psicoterapia psicodinâmica de longa duração. Método: num estudo naturalista de psicoterapia psicodinâmica de longa duração(n = 53), 31 clientes relataram ideação suicida, 8 dos quais também reportaram tentativas de suicídio anteriores, enquanto 22 eram não-suicidas. A ideação e tentativas de suicídio e as componentes da aliança terapêutica foram seguidas em intervalos de 6 meses.

Resultados: os clientes suicidas tinham mais diagnósticos psiquiátricos, foram tratados durante mais tempo e reportaram mais reações negativas ao tratamento do que os clientes não suicidas.

A aliança moderou (a) a relação entre reacção negativa ao tratamento e melhoria da ideação suicida e (b) a relação entre a qualidade da interacção cliente-terapeuta e a melhoria.

Conclusões: clientes suicidas manifestam hostilidade e reacções negativas ao tratamento, indicando transferência negativa. Evitar os erros do terapeuta que resultam da contratransferência negativa melhora a interacção terapêutica, o que se encontra associado a uma recuperação mais rápida dos fenómenos suicidas.

Gegenstand: Wir untersuchten therapeutische Prozesse, welchemit Veränderungen in der Suizidalität in einer naturalistischen Studie zur Langzeit Psychodynamischen Therapie verbunden waren. Methoden: In einer naturalistischen Studie zur Langzeit Psychodynamischen Therapie (n = 53) zeigten 31 Patienten Suizidgedanken, von denen 8 vorherige Suizidversuche berichteten, während 22 Patienten der Studie nicht suizidal waren. Suizidgedanken und –versuche, sowie Komponenten psychotherapeutischer Allianz wurden in Abständen von 6 Monaten erhoben. Ergebnisse: Suizidale Patienten hatten mehr psychiatrische Diagnosen, wurden länger behandelt und berichteten mehr negative Reaktionen in Bezug auf die Behandlung als nicht suizidale Patienten. Die therapeutische Allianz moderierte (a) den Zusammenhang zwischen negativen Reaktionen in Bezug auf die Behandlung und Suizidgedanken und (b) den Zusammenhang zwischen der Qualität der Patienten-Therapeuten Interaktion und therapeutischer Verbesserung. Schlussfolgerungen: Suizidale Patienten entwickelten Feindseligkeit und negative Reaktionen in Bezug auf die Behandlung, was negative Übertragungsprozesse indiziert. Die Vermeidung von therapeutischen Fehlern, welche aus negativen Gegenübertragungsprozessen hervorgeht verbessert die therapeutischen Interaktionen, was zu einer schnelleren Verbesserung der Suizidalität führt.

Obiettivo: abbiamo studiato i processi terapeutici associati ad un miglioramento nella tendenza al suicidio in uno studio naturalistico di una psicoterapia dinamica a lungo termine.

Metodo: in uno studio naturalistico con una psicoterapia dinamica a lungo termine (n_53), 31 pazienti riportarono ideazione suicidaria, otto di questi riferirono, inoltre, precedenti tentativi di suicidio, mentre 22 furono classificati come non-suicidari. L'ideazione suicidaria, i tentativi e le componenti dell'alleanza terapeutica sono stati osservati per 6 mesi. Risultato: I pazienti suicidari mostrarono un maggior numero di diagnosi psichiatriche, sono stati trattati più a lungo e riportarono più reazioni negative al trattamento dei pazienti non-suicidari. L'alleanza modera (a) la relazione tra le reazioni negative al trattamento e un miglioramento nell'ideazione suicidaria e (b) la relazione tra la qualità dell'interazione paziente-terapeuta e il miglioramento.

Conclusioni: I pazienti con tendenze suicidarie manifestano ostilità e reazione negativa al trattamento, e ciò indica la presenza di transfert negativo. Evitare nei terapeuti gli errori che nascono da un controtransfert negativo migliora le interazioni terapeutiche, che sono poi seguite da un più rapido miglioramento nella suicidalità.

目的：我們使用自然研究法研究自殺病人在長期動力心理治療歷程中之進展。 研究法：在使用自然研究法之長期動力心理治療研究中(參與者人數 = 53)， 31 名病人表明有自殺意念，其中八人曾有過自殺企圖，而另外22人無自殺意念。 六個月後研究者針對研究參與者之自殺意念及企圖、治療同盟成分予以追蹤。 結果：有自殺意念之病人相較於無自殺意念之病人之下，有較多之精神疾患診 斷、曾接受較長治療、且自述對於治療有較多負向反應。同盟關係調節了(a) 對治療之負向反應及降低自殺意念之間的關係，以及(b)病人-治療師互動的品 質和治療進展之間的關係。結論：有自殺意念的病人對於治療有敵意及負向反 應，顯示病人的負向移情。避免治療師在負向反移情所造成的錯誤能增進具有 療癒的互動，由此能促進治療自殺的進展。
