A tripartite model of the therapeutic relationship: Theory, research, and practice

Abstract

The development and empirical examination of a tripartite model of the therapeutic relationship over nearly three decades are described. The model asserts that all therapeutic relationships, to varying degrees, consist of a real relationship, a working alliance, and a transference-countertransference configuration. Research testing propositions about how each of these components is related to treatment process and outcome, and to each other, is presented. Many propositions have been supported, but some have been disconfirmed. Although the tripartite, or perhaps a quadripartite, model appears to be empirically and theoretically viable, continued research and theoretical development will serve to refine the model further. The development and testing of additional models that unpack the global concept of the therapeutic relationship would also be useful.

In questo lavoro viene descritto lo sviluppo e la sperimentazione del modello tripartito della relazione terapeutica di riferimento negli ultimi trent'anni. Secondo il modello tutte le relazioni terapeutiche, pur variando di grado, consistono in una relazione autentica, un'alleanza di lavoro e in un'organizzazione di transfert e controtransfert. Viene presentata la ricerca che indaga l'ipotesi in base alla quale ognuna di queste componenti è correlata al trattamento e all'esito, e ciascuna all'altra. Alcune ipotesi sono state confermate, altre invece non hanno trovato riscontro empirico. Anche se il modello tripartito, o forse quadripartito, sembra essere teoricamente e empiricamente valido, la continua ricerca e lo sviluppo della teoria a monte servirà per affinare i modelli futuri. Potrebbero essere di valido aiuto lo sviluppo e la validazione di modelli supplementari che dettaglino ulteriormente il concetto macroscopico di relazione terapeutica.

É descrito o desenvolvimento e estudo empírico ao longo de quase três décadas de um modelo tripartido da relação terapêutica. O modelo assume que todas as relações terapêuticas, em níveis diversos, consistem numa relação real, de uma aliança de trabalho e de um padrão de transferência-contratransferência. São apresentadas as proposições testadas pela investigação acerca de como cada um destes componentes se relaciona com o processo e o resultado do tratamento, e entre si. Muitas proposições foram suportadas mas algumas foram infirmadas. Embora um modelo tripartido, ou talvez quadripartido, pareça empírica e teoricamente viável, a investigação e o desenvolvimento teórico futuros servirão para refinar o modelo. O desenvolvimento e teste de modelos adicionais que desmantelem o conceito global de relação terapêutica seriam igualmente úteis.

Die Entwicklungen und empirischen Untersuchungen der letzten drei Dekaden zum dreiteiligen Model der therapeutischen Beziehung werden beschrieben. Das Modell behauptet, dass alle therapeutischen Beziehungen, zu unterschiedlichen Ausmaßen, aus einer realen Beziehung, einer Arbeitsallianz und einer Übertragungs-Gegenübertragungs-Konfiguration bestehen. Es wird die Forschung vorgestellt, welche Aussagen bezüglich der wechselseitigen Beeinflussung der einzelnen Komponenten und ihre Einfluss auf den Behandlungsprozess und sein Ergebnis überprüft. Viele Aussagen fanden Unterstützung, einige wurden allerdings widerlegt. Obwohl das dreiteilige, oder sogar vierteilige, Modell empirisch und theoretisch praktikabel erscheint, braucht es weitergehende Forschung und theoretische Entwicklungen um das Modell weiter zu spezifizieren. Die Entwicklung und Überprüfung zusätzlicher Modelle, welche das globale Konzept der therapeutischen Beziehung herunterbrechen wären ebenfalls hilfreich.

本文介紹已經發展將近三十年的治療關係三元模式及其實證性的研究資料。該模式認為所有的治療關係都包含了不同程度的真實關係、治療同盟以及移情－反移情的成分。有許多文獻都在探究這三個成分與處遇歷程及結果的關聯性，或是此三個成分間的關係。有許多假設都得到支持，但是也有些假設被拒絕。雖然此三元模式，或是稱為四元模式，不管在實證研究或是理論上都頗受肯定，但是仍須有理論與研究繼續探究與發展此模式。此外發展或檢視其他有關治療關係概念的其他模式也將有所助益。
