Patient Attachment to Therapist Rating Scale: Development and psychometric properties

Abstract

Objective: To report on the development and initial psychometric properties of a new rating scale for patent-therapist attachment. Method: Seventy interviews from the Young Adult Psychotherapy Project (YAPP) were rated. Results: Excellent internal consistency (Cronbach's α > .90) was observed for all four subscales (Security, Deactivation, Hyperactivation, and Disorganization). Three subscales showed good inter-rater reliability (ICC > .60), while one (Hyperactivation) had poor (ICC < .40). Correlations with measures of alliance, mental representations, and symptom distress support the construct validity of the reliable subscales. Exploratory factor analysis indicated three underlying factors explaining 82% of the variance. Conclusions: The Patient Attachment to Therapist Rating Scale is a promising approach for assessing the quality of attachment to therapist from patient narratives. Future development should focus on improving the discrimination of the insecure subscales.

Obiettivo: Descrivere lo sviluppo e le iniziali proprietà psicometriche di una nuova scala di valutazione per l'attaccamento paziente-terapeuta.

Metodo: Sono state valutate settanta interviste effettuate nell'ambito del Young Adult Psychotherapy Project (YAPP).

Risultati: E’ stata osservata una coerenza interna eccellente (Cronbach α > .90) per tutte e quattro le sottoscale (sicurezza, disattivazione, iperattivazione, disorganizzazione). Tre sottoscale hanno mostrato una buona affidabilità tra i valutatori (ICC> .60), mentre una sottoscala (iperattivazione) ha avuto un punteggio basso (ICC <.40). Le correlazioni con le misure di alleanza, rappresentazioni mentali, e sintomi di sofferenza confermano la validità di costrutto delle sottoscale affidabili. L’analisi fattoriale esplorativa ha indicato tre fattori sottostanti che spiegano l'82% della varianza.

Conclusioni: La Patient Attachment to Therapist - Rating Scale si è dimostrata promettente per valutare la qualità di attaccamento col terapeuta a partire dalle narrazioni del paziente. Sviluppi futuri dovrebbero concentrarsi sul miglioramento della discriminazione delle sottoscale meno sicure.

Objectivos: Relatar o desenvolvimento e propriedades psicométricas iniciais de uma nova escala de avaliação da vinculação paciente-terapeuta. Método: Setenta entrevistas do Projeto de Psicoterapia para Jovens Adultos (YAPP) foram avaliadas. Resultados: Foi observada uma excelente consistência interna (α de Cronbach > .90) para as quatro subescalas do instrumento (Segurança, Desativação, Hiperativação e Desorganização). Três subescalas apresentaram boa fidelidade inter-avaliadores (ICC > .60) enquanto uma (Hiperativação) apresentou uma baixa fidelidade (ICC > .40). Correlações com medidas de aliança, representações mentais e intensidade dos sintomas apoiam a validade de construto das subescalas com boa fidelidade. A análise factorial exploratória indicou três fatores subjacentes que explicam 82% da variância. Conclusões: A Escala de Avaliação da Vinculação do Paciente ao Terapeuta é uma abordagem promissora para avaliar a qualidade da vinculação ao terapeuta através das narrativas dos pacientes. Desenvolvimentos futuros devem concentrar-se em melhorar a discriminação das subescalas inseguras.

Ziel: Von der Entwicklung und den anfänglichen psychometrischen Eigenschaften einer neuen Bewertungsskala für Patient-Therapeuten Bindung zu berichten.

Methode: Es wurden siebzig Interviews aus dem Young Adult Psychotherapy Project (YAPP) bewertet.

Ergebnis: Exzellente interne Konsistenzen (Cronbachs α > .90) wurden für alle vier Subskalen festgestellt (Sicherheit, Deaktivierung, Hyperaktivierung und Desorganisation). Drei Subskalen zeigten gute Interrater-Reliabilität (ICC > .60), währen eine (Hyperaktivierung) unzureichend ausfiel (ICC < .40). Korrelationen mit Allianzmaßen, mentaler Repräsentation und Symptombelastung unterstützen die Konstruktvalidität der reliablen Subskalen. Eine explorative Faktorenanalyse zeigte drei zugrundeliegende Faktoren, welche 82% der Varianz aufklärten.

Schlussfolgerung: Die Patient Attachment to Therapist Rating Scale ist ein vielversprechender Ansatz, um die Qualität der Bindung des Patienten zum Therapeuten anhand von Patientennarrativen zu erheben. Zukünftige Entwicklungen sollten sich auf die Verbesserung der Unterscheidung der unsicheren Subskalen konzentrieren.

目的：提出一份病人對治療師依附之新評量表的建立與初步心理計量屬性。方法：對青年心理治療計畫(YAPP)中的七十份訪談稿進行評量。結果：四個分量尺（安全感、低活性、高活性、失序）皆有優良的內部一致性(Cronbach ’ s α > .90)；三個分量尺有不錯的評分者間信度(ICC > .60)，但一個分量尺（高活性）的評分者間信度不佳(ICC < .40)。本量表與同盟關係、心理表徵、症狀不適程度的相關，皆支持信度佳的分量尺具有建構效度。探索性因素分析顯示三個潛在因素可解釋82%之變異量。結論：病人對治療師依附評量表應是由病人之敘述評估其對治療師之依附品質的可行方法，未來應再致力於改善不安全分量尺之區辨性。
