The associations among improvement and alliance expectations, alliance during treatment, and treatment outcome for major depressive disorder

Abstract

Objective: To examine the associations between treatment/outcome expectations, alliance before and during treatment, and the impact of alliance on symptomatic improvement. Methods: One hundred and fifty-three depressed patients randomized to dynamic supportive-expressive psychotherapy (SET), antidepressant medication (ADM) or placebo (PBO) + clinical management completed ratings of treatment expectations, therapeutic alliance (CALPAS, WAI-S), and depressive symptoms (HAM-D). Results: Pretreatment expectations of the therapeutic alliance were significantly related to alliance later in therapy but did not differ across treatments and did not predict outcome. Alliance development over time differed between treatments; it increased more in SET than in PBO. After controlling for prior symptom improvement, early alliance predicted subsequent depression change. Conclusions: Expectations of alliance and of treatment outcome/improvement, measured prior to treatment onset, predicted subsequent alliance.

Obiettivo: Esaminare le associazioni tra le aspettative circa il trattamento/esito, l'alleanza prima e durante il trattamento e l'impatto dell'alleanza sul miglioramento dei sintomi. Metodi: 153 pazienti depressi randomizzati in trattamenti con psicoterapia dinamica supportiva-espressiva (SET), farmaci antidepressivi (ADM) o placebo (PBO) + gestione clinica hanno completato valutazioni relative alle aspettative di trattamento, l'alleanza terapeutica (CALPAS, WAI-S) e i sintomi depressivi (HAM-D). Risultati: le aspettative pretrattamento circa l'alleanza terapeutica erano significativamente legate all'alleanza sviluppata più tardi nel corso della terapia, ma non differivano tra i diversi trattamenti e non erano predittive degli esiti. Lo sviluppo dell'alleanza nel tempo differiva invece a seconda del trattamento: migliorava più nella SET che nella PBO. Dopo il controllo del miglioramento dei sintomi precedenti, l'alleanza precoce riusciva a predire la variazione della depressione successiva. Conclusioni: le aspettative di alleanza e di esito/miglioramento nel trattamento, misurate prima dell'inizio del trattamento, davano indicazioni sull'alleanza successiva.

Objetivo: Examinar as associações entre as expectivas de tratamento/resultado, a aliança antes e durante o tratamento e o impacto da aliança na melhoria sintomática. Método: Cento e cinquenta e três pacientes deprimidos, divididos de forma aleatória randomizada pelas condições psicoterapia dinâmica de suporte-expressiva (“dynamic supportive-expressive psychotherapy“,SET), medicação antidepressiva (ADM) ou placebo (PBO) + seguimento clínico completaram medidas de expectativas de tratamento, aliança terapêutica (CALPAS, WAI-S) e sintomas depressivos (HAM-D). Resultados: As expectativas pré-tratamento da aliança terapêutica estavam significativamente relacionadas com a aliança em fases posteriores da terapia mas não diferiam em diferentes tratamentos e não prediziam os resultados terapêuticos. O desenvolvimento da aliança ao longo do tempo diferiu entre os tratamentos; aumentou mais na SET do que no PBO. Depois de se controlar a melhoria sintomática prévia, a aliança inicial revelou-se preditor da mudança depressiva subsequente. Conclusões: As expectativas de aliança e de resultado/melhoria do tratamento, medidos antes do início do tratamento, foram capazes de predizer a aliança subsequente.

Ziel: Die Zusammenhänge zwischen Behandlungs-/Ergebniserwartungen, Allianz vor und nach der Behandlung und den Einfluss von Allianz auf die Symptomverbesserung zu untersuchen. Methoden: 153 depressive Patienten, randomisiert zu dynamisch unterstützend-expressiver Psychotherapie (SET), Antidepressivum Medikation (ADM) oder Placebo (PBO) + klinischem Management bewerteten Behandlungserwartungen, therapeutische Allianz (CALPAS, WAI-S) und depressive Symptome (HAM-D). Ergebnisse: Erwartungen vor der Behandlung zur therapeutischen Allianz hingen signifikant mit Allianz später in der Therapie zusammen, aber unterschieden sich nicht zwischen den Behandlungen und sagten nicht das Ergebnis vorher. Die Entwicklung der Allianz über die Zeit unterschied sich zwischen den Behandlungen; sie nahm stärker in SET als in PBO zu. Nach Kontrolle für vorherige Symptomverbesserungen, sagte frühe Allianz anschließende Depressionsveränderung voraus. Schlussfolgerung: Erwartungen zur Allianz und von Behandlungsergebnis /-verbesserung, gemessen vor Behandlungsbeginn, sagt nachfolgende Allianz voraus.
