Abstract
Objective: We investigated the association between socio-demographic and clinical variables with mental health care stigma, and the impact of the latter on the quality of the therapeutic alliance measured at intake. Method: Consecutive clients (N = 236) filled questionnaires upon accessing services for a new episode of care. Immediately following the intake, a randomly selected sample of clients and their corresponding therapists (n = 102) completed the Working Alliance Inventory – Bond Scale. Results: Lower mean years of education and higher emotional distress (both partial r = .17) were significantly associated with higher stigma. Higher care stigma negatively correlated with therapists' ratings of the therapeutic alliance during the intake (partial r = −.22), but not with those of clients. Conclusions: Care stigma is present among service-users and may affect outcomes of the intake.
Obiettivo: Abbiamo studiato l'associazione tra le variabili socio-demografiche e cliniche con lo stigma sulla cura della salute mentale, e l'impatto di queste sulla qualità dell'alleanza terapeutica misurata al primo contatto. Metodo: Tutti i pazienti (N = 236), in successione, hanno riempito dei questionari nel momento in cui accedevano ai servizi per una nuova seduta di trattamento. Immediatamente dopo il primo contatto, un campione scelto a caso di pazienti e i loro terapeuti (n = 102) hanno completato la Working Alliance Inventory – Bond Scale. Risultati: Un valore medio più basso di anni di istruzione e maggior stress emozionale (r parziale in entrambi = .17) erano associati in modo significativo con maggiore stigma. Maggiore stigma per la cura correlava negativamente con le valutazioni dei terapeuti dell'alleanza terapeutica durante il primo contatto (r parziale = –.22), ma non con quelle dei pazienti. Conclusioni: Lo stigma per la cura è presente tra gli utenti del servizio e può influenzare l'esito del primo contatto.
Ziel: Wir haben den Zusammenhang zwischen soziodemografischen und klinischen Variablen mit dem Stigma der psychischen Gesundheitsversorgung und den Auswirkungen letzterer auf die Qualität der therapeutischen Allianz bei der Aufnahme gemessen. Methode: Konsekutive Klienten (N = 236) füllten bei der Inanspruchnahme von Serviceleistungen für eine neue Versorgungsepisode Fragebögen aus. Unmittelbar folgend auf die Aufnahme füllte eine randomisiert ausgewählte Stichprobe und ihre dazugehörigen Therapeuten (n = 102) das Working Alliance Inventory – Bindungsskala aus. Ergebnisse: Niedrigere mittlere Jahre an Bildung und größere emotionale Belastung (beide partielles r = .17) hingen beide signifikant mit einem höheren Stigma zusammen. Ein höheres Versorgungsstigma korreliert negativ mit den Therapeutenbewertungen der therapeutischen Allianz während der Aufnahme (partielles r = –.22), aber nicht mit denen der Klienten. Schlussfolgerung: Das Versorgungsstigma ist präsent unter Servicenehmern und könnte das Ergebnis der Aufnahme beeinflussen.
Objetivo: Investigámos a associação entre variáveis clínicas e sócio-demográficas e o estigma dos cuidados de saúde mental, e o impacto deste último na qualidade da aliança terapêutica medida na admissão. Método: Clientes consecutivos (N = 236) preencheram questionários à medida que acediam aos serviços pora um novo episódio de cuidados. Imediatamente após a admissão, uma amostra aleatoriamente selecionada de clientes e respetivos terapeutas (N = 102) completaram o Inventário de Aliança Terapêutica – Escala “Bond”. Resultados: Menos anos de formação e maior mal-estar emocional (ambos parcial r = .17) mostraram-se significativamente associados a maior estigma. Um maior estigma em relação aos cuidados revelou-se negativamente correlacionado com as pontuações dos terapeutas ao nível da aliança terapêutica durante a admissão (parcial r = –.22) mas não com as pontuações dos clientes. Conclusões: O estigma em relação aos cuidados está presente entre os usuários dos serviços e pode afectar os resultados da admissão.
