Abstract
Objective: This paper reviews the construct of the working alliance, beginning with its historical development and moving into its modern pantheoretical conceptualization. Method: Major research efforts on the topic are reviewed. The review includes both theoretical and empirical literature on the working alliance and related constructs, such as alliance ruptures and repair, and therapeutic negotiation. Existing measures of the working alliance are also reviewed. Results: The working alliance is critiqued on both theoretical and empirical grounds, including the strengths and limitations of current approaches and assessments. Recent theoretical developments are reviewed, among them work on alliance rupture and repair and the reconceptualization of the alliance as a process of therapeutic negotiation. Emerging applications of advanced statistical techniques to measure the working alliance are also considered. Conclusion: The review concludes by supporting recent efforts that have attempted to increase the sophistication of measurement tools and statistical approaches, and encouraging future research in these areas.
Obiettivo: Questo articolo passa in rassegna il costrutto dell'alleanza terapeutica, a cominciare dal suo sviluppo storico spostandosi poi alla sua più moderna concettualizzazione panteoretica. Metodo: sono state esaminate le principali ricerche sull'argomento. La review include sia la letteratura teorica ed empirica sull'alleanza di lavoro e sui costrutti correlati, come rotture e riparazione dell'alleanza, e la negoziazione terapeutica. Sono state riviste anche le misure esistenti dell'alleanza terapeutica. Risultati: E’ stata analizzata l'alleanza di lavoro su un piano sia teorico che empirico, inclusi i punti di forza e i limiti degli approcci attuali e l'assessment. Sono stati rivisti i recenti sviluppi teorici, tra i quali il lavoro sulla rottura e riparazione dell'alleanza e la riconcettualizzazione dell'alleanza come un processo di negoziazione terapeutica. Sono state considerate anche le applicazioni emergenti di tecniche di statistica avanzata per misurare l'alleanza di lavoro. Conclusione: In conclusione, la review sostiene gli sforzi recenti che hanno tentato di aumentare la complessità di strumenti di misura e di approcci statistici, incoraggiando la ricerca futura in questi settori.
Ziel: Diese Studie betrachtet das Konstrukt der therapeutischen Allianz, beginnend bei der historischen Entwicklung bis zur modernen pantheoretischen Konzeptualisierung. Methode: Die wichtigsten Bestrebungen bei der Erforschung dieses Themas werden betrachtet. Diese Betrachtung umfasst sowohl die theoretische als auch die empirische Literatur zur therapeutischen Allianz und verwandten Konstrukten wie etwa Beziehungsbrüche und –reparaturen sowie die therapeutische Gesprächsführung. Ebenso werden bereits bestehende Maße der therapeutischen Allianz betrachtet. Ergebnisse: Die therapeutische Allianz wird sowohl aus theoretischer als auch aus empirischer Sicht rezensiert, einschließlich der Stärken und Schwächen aktueller Ansätze und Erfassungsinstrumente. Es werden die jüngsten theoretischen Entwicklungen, unter anderem Arbeiten zu Beziehungsbruch und-reparatur sowie zur Neukonzeptualisierung der Allianz als Prozess der therapeutischen Gesprächsführung geprüft. Außerdem werden neue Anwendungen fortschrittlicher statistischer Techniken zur Messung der therapeutischen Allianz betrachtet. Schlussfolgerung: Die Review unterstützt neuste Bestrebungen zur Verfeinerung der Messinstrumente und statistischen Ansätze und regt zu zukünftiger Forschung in diesen Bereichen an.
Objetivo: Este artigo apresenta uma revisão sobre o constructo da aliança terapêutica, desde o seu desenvolvimento histórico à sua concetualização transteórica mais atual. Método: Foram revistos os principais trabalhos de investigação realizados sobre o tema. A revisão inclui literatura teórica e empírica sobre a aliança terapêutica e constructos relacionados, tais como ruturas e reparações da aliança e negociação terapêutica. Foram também revistas as medidas de aliança terapêutica existentes. Resultados: A aliança terapêutica é discutida quer do ponto de vista teórico quer empírico, incluindo os pontos fortes e as limitações das abordagens e medidas atuais. Foram revistos os mais recentes desenvolvimentos teóricos, de entre os quais o trabalho efetuado sobre ruturas e reparações da aliança, e a reconceptualização da aliança como processo de negociação terapêutica. Foram ainda consideradas as aplicações emergentes de técnicas estatísticas avançadas na avaliação da aliança terapêutica. Conclusão: Esta revisão conclui-se apoiando os esforços recentes que têm tentado aumentar a sofisticação das ferramentas de avaliação e das abordagens estatísticas, e encorajando futuras investigações nestas áreas.
目的：本文回顧工作同盟的建構，先由它的歷史發展著手，進而討論其當代的泛理論概念架構。方法：回顧此議題的重要研究工作，回顧範圍包括工作同盟之理論性與實徵性文獻，例如，同盟破裂與修復，以及治療協商，亦一併回顧工作同盟的現有測量。結果：對工作同盟的理論面與實徵依據加以評述，包括它在當今作法與評估上的優點與限制。所回顧的近期理論發展中，包括有同盟破裂與修復方面的努力，以及將同盟重新概念化為「治療性協商的歷程」。另外，也發現有逐漸以進階統計方法測量工作同盟的趨勢。結論：本回顧肯定工作同盟近期在提升測量工具與統計方法之精細程度上的努力，並期許此領域有進一步的研究。

1 Transference has been historically defined as the displacement of feelings, attitudes, or defenses toward a person in the present (e.g., the therapist) which are inappropriate and originate in the past, usually in regard to a significant person in the patient's early life (Freud, Citation1912). Freud distinguished between positive and negative transference, and referred to the therapeutic relationship in the context of the former.
