Predicting outcome of substance abuse treatment in a feedback study: Can recovery curves be improved upon?

Abstract

Objective: The goal of the study was to evaluate whether enhanced normative feedback recovery curves are needed for treatment of substance use problems. Method: Patient predictors of outcome were examined using data from four substance abuse treatment clinics. Results: Baseline severity of symptoms/functioning, employment, and craving were found to be associated with rate of change in symptoms/functioning. Several other variables were associated with rate of change in alcohol use, although in the opposite direction than found in efficacy trials. Conclusions: The results point to the complexity of designing feedback systems using normative recovery curves for those with substance use problems and highlight the important differences between real-world treatment of those with substance use problems compared to data from efficacy trials.

Obiettivo: L'obiettivo dello studio era quello di valutare se le curve normative avanzate di guarigione formulate a partire dal feedback siano necessarie per il trattamento dei problemi da uso di sostanze. Metodo: sono stati esaminati predittori di esito dei pazienti utilizzando i dati ricavati da quattro ambulatori per il trattamento dell'abuso di sostanze. Risultati: è emerso che la severità iniziale dei sintomi/funzionamento, l'occupazione e il craving erano associati con il tasso di cambiamento dei sintomi/funzionamento. Sono state associate diverse altre variabili con il tasso di cambiamento nell'uso di alcol, anche se in direzione opposta a quella trovata in trials di efficacia. Conclusioni: I risultati mostrano la complessità nel progettare sistemi di feedback utilizzando le curve normative di recovery in soggetti con problemi da uso di sostanze ed evidenziano le differenze importanti tra il trattamento nel mondo reale di questi pazienti rispetto ai trials di efficacia.

Zusammenfassung:Ziel: Das Ziel dieser Studie war zu bewerten, ob erweiterte normative Feedback-Erholungskurven für die Behandlung von Substanzmissbrauch benötigt werden. Methode: Patientenvariablen wurden als Prädiktoren der Behandlungsergebnisse mittels Daten von vier Fachkliniken für Substanzmissbrauch untersucht. Ergebnisse: Die Schwere der Symptomatik bzw. Funktionsbeeinträchtigung zu Beginn, das Beschäftigungsverhältnis sowie das Verlangen zeigten sich als mit der Veränderungsrate der Symptomatik bzw. des Funktionierens assoziiert. Einige weitere Variablen waren mit der Veränderungsrate bzgl. des Alkoholkonsums assoziiert, obwohl die Ergebnisse von Efficacy Untersuchungen in die andere Richtung zeigten. Schlussfolgerungen: Die Ergebnisse zeigen die Komplexität der Gestaltung von Feedbacksystemen unter Verwendung von normativen Erholungskurven bei Patienten mit Substanzmissbrauch auf und betonen die wichtigen Unterschiede zwischen der realen Behandlung von Patienten mit Substanzmissbrauch und Daten der Efficacy-Forschung.

Objetivo: O objetivo do estudo foi avaliar se as curvas de recuperação de feedback normativo avançado são necessárias ao tratamento de problemas de uso de substâncias. Método: Preditores de resultado dos pacientes foram analisados através do uso de dados de quatro tratamentos clínicos de abuso de substâncias. Resultados: A severidade da baseline dos sintomas/funcionamento, o emprego, e a necessidade de consumo estavam associados à taxa de variação nos sintomas/funcionamento. Diversas outras variáveis foram associadas à taxa de alteração quanto ao uso de álcool, embora na direção oposta àquela encontrada em ensaios de eficácia. Conclusões: Os resultados apontam para a complexidade de projetar sistemas de feedback que usem curvas de recuperação normativas para sujeitos com problemas de uso de substâncias, e destacam as importantes diferenças que existem entre o tratamento no mundo real das pessoas com problemas de uso de substâncias, quando comparado aos dados de ensaios de eficácia.

目標:這項研究之目標在評量對於物質濫用問題之處遇，是否需要用到強化的基準回饋復原曲線。方法:檢測來自四個物質濫用治療診所病患的成效預測值資料。結果:症狀/功能之嚴重程度基準線，就業情形，以及上癮的程度與症狀/功能之改變情形有關。有幾個其他的變項也和酒精濫用的改變情形有關，雖然和其他效果研究的發現大相徑庭。結論:結果指出為物質濫用問題使用基準復原曲線設計回饋系統，有其複雜度；且凸顯真實世界中物質濫用問題的處遇和效果研究所獲得的資料有極大的不同。
