Abstract
Objective: Little is known about how therapy processes relate to outcome in cognitive–behavioral therapy (CBT) for panic disorder (PD). This study examined whether client resistance predicts CBT for PD outcomes beyond the effects of established pre-treatment predictors. A secondary aim was to assess the consistency of resistance over treatment. Method: Data were from 71 adults participating in up to 24 biweekly sessions of CBT in a randomized controlled trial. Panic severity was assessed before, during (at Weeks 1, 5, and 9), and at termination of treatment (Week 12) using the Panic Disorder Severity Scale. Trained coders reliably rated resistance in videos of Sessions 2 and 10 using the Client Resistance Code. Results: Resistance was found to be moderately consistent (r = .64). Although overall resistance was unrelated to outcomes, hierarchical linear modeling revealed that openly hostile resistance at Session 10 predicted significantly diminished symptom change (r = .28, CI95% = [.01, .51]), beyond the effects of pretreatment predictors. Hostile resistance at Session 2 predicted attrition (rrb = −.30, p = .001), even after established predictors were controlled. Conclusions: Although some forms of resistance may be benign, openly hostile resistance is an important therapy marker that warrants increased clinical and research attention.
Obiettivo: si sa ancora poco su come i processi terapeutici siano correlati ai risultati della terapia cognitivo-comportamentale (CBT) per il disturbo da panico (PD). Questo studio ha esaminato se la resistenza del cliente predice gli esiti del PD oltre ai predittori di pre-trattamento stabiliti. Un obiettivo secondario era valutare la costanza della resistenza rispetto al trattamento. Metodo: I dati provenivano da 71 adulti che partecipavano a un massimo di 24 sedute bisettimanali di CBT in uno studio controllato randomizzato. La gravità del panico è stata valutata prima, durante (alla settimana 1, 5 e 9) e al termine del trattamento (settimana 12) usando il Panic Disorder Severity Scale. I programmatori addestrati hanno valutato in modo affidabile la resistenza nei video delle sedute 2 e 10 utilizzando il Client Resistence Code. Risultati: la resistenza è risultata moderatamente costante (r=.64). Sebbene la resistenza complessiva fosse non correlata ai risultati, il modello gerarchico lineare ha rivelato che la resistenza apertamente ostile alla Seduta 10 prediceva una diminuizione significativa dei sintomi (r=.28, CI95%=[.01, .51]), oltre agli effetti dei predittori di pretrattamento. Objetivo: Pouco se sabe sobre como os processos de terapia se relacionam com os resultados da terapia cognitivo-comportamental (TCC) para o transtorno do pânico (TP). Este estudo examinou se a resistência do paciente prediz os resultados da TCC para o TP para além dos efeitos dos preditores estabelecidos antes do tratamento. Um objetivo secundário foi avaliar a consistência da resistência ao longo do tratamento. Método: Os dados foram de 71 adultos participantes de até 24 sessões quinzenais de TCC em um estudo controlado randomizado. A gravidade do pânico foi avaliada antes, durante (nas semanas 1, 5 e 9) e no término do tratamento (semana 12) usando a Panic Disorder Severity Scale. Codificadores treinados classificaram a resistência nos vídeos das Sessões 2 e 10 usando o Client Resistance Code. Resultados: a resistência foi moderadamente consistente (r=0,64). Embora a resistência geral não tenha relação com os resultados, a modelagem linear hierárquica revelou que a resistência abertamente hostil na Sessão 10 previu, significativamente, na mudanças dos sintomas (r=0,28, IC95%=[01, 51]), além dos efeitos dos preditores de pré-tratamento. A resistência hostil na Sessão 2 previu atrito (rrb=- 0,30; p=0,001), mesmo após o controle dos preditores estabelecidos. Conclusões: Embora algumas formas de resistência possam ser benignas, a resistência abertamente hostil é um importante marcador terapêutico que merece maior atenção clínica e de pesquisa.
