Jerry Spiegel is Director of Global Health at the Liu Institute for Global Issues and Associate Professor in the School of Population and Public Health at the University of British Columbia. He teaches courses on Global Health and Human Security and International Aid, Trade and Health, and directs research programs in Cuba, Ecuador, and South Africa on the management of health determinants and related governance issues. Robert Huish is Assistant Professor in International Development Studies at Dalhousie University where he teaches courses on Global Health, Poverty and Human Rights, Development, and Activism. His current research interests include the ethics of medical education, health care worker migration, and social tolerance for massive inequities in health care provision. This article builds on an analysis of the subject that the authors conducted for Global Health Watch 2, An Alternative World Health Report, in collaboration with Andrea Cortinois and Klaudia Dmitirienko. (GHW 2008).
