When the above article was first published online, the in-text citations on page 2 paragraph 2 were listed in error as follows:

  1. Blatter and Collier 2010, 2011 instead of Collier 2010, 2011

  2. Haverland 2012 instead of Haverland and Blatter 2012

These have now been corrected in both the online and print versions.
