Current incidence and management of children with traumatic head injuries: The Stockholm experience

Abstract

Purpose: A traumatic head injury is one of the most common causes of morbidity and mortality among children, however few population-based studies in this area have been reported. Therefore, the aim of this study was to evaluate the incidence and management of traumatic head injuries in children at a level-one trauma centre in Stockholm, Sweden.

Participants: All children (n = 3168) who visited the emergency department with a history of head injury during 1 year were included.

Method: The required information was collected retrospectively and the children's medical records were reviewed.

Results: The overall incidences of head injury were 865 per 100 000 children with the highest incidence (2379/105 children) occurring among children younger than 18 months of age. Twelve per cent (n = 396) were admitted to a hospital ward and CT scans were performed in 13% (n = 412) of all children. During this year, 0.3% required a neurosurgical intervention and only 1% of all children had documentation of a planned follow-up appointment.

Conclusion: The findings indicate that clinical documentation as a part of the early management in children with a head injury is inconsistent and suffers from lack of valid criteria. Implementation of clinical guidelines during emergency care would help improve subsequent hospital care, as well as the planning of health care services for these children.

El trauma craneoencefálico es una de las causas más comunes de morbilidad y mortalidad entre los niños, sin embargo pocos estudios basados en la población han sido reportados en esta área. Por lo tanto el objetivo de este estudio fue evaluar la incidencia y el manejo de los traumatismos craneoencefálicos en niños en el centro de trauma de nivel 1 en Estocolmo Suecia.

Participantes: se incluyeron a todos los niños (n = 3168) que acudieron durante un año al departamento de urgencias con una historia de traumatismo craneoencefálico.

Método: la información requerida fue recolectada en forma retrospectiva y se revisaron los registros médicos de los niños.

Resultados: en total la incidencia de traumatismo craneoencefálico fue de 865 por 100,000 niños. Teniendo lugar la mayor incidencia (2379/10 5 niños) entre los niños con una edad menor a 18 meses. Se admitieron 12% (n = 396) a la sala del hospital y se realizaron tomografías computarizadas en 13% (n = 412) del total de los niños. Durante este año 0.3% requirieron una intervención neuroquirurgica y solamente 1% de los niños tenían documentada una cita planeada de seguimiento.

Conclusión: nuestros hallazgos indican que la documentación clínica como parte del manejo temprano en los niños con TCE es inconsistente y carece de criterios validos. La implementación de guías clínicas durante el manejo de emergencia ayudará a mejorar el manejo hospitalario subsecuente, así como la planeación de los servicios de cuidados de salud para estos niños.
