Service users’ perceptions of shared agency in mental health services

Abstract

Objective: The concept of shared agency is used in examining the collaboration between the service user and the professionals. The study aims to find out what kind of experiences on shared agency mental health service users have.

Method: The study was based on interviews of 19 service users. The data set was formed of those parts of the interviews that dealt with shared aims, being heard, collaboration and joint planning and decision-making. Typical methods of inductive content analysis were applied.

Results: Three domains of shared agency were found: emotional, communicative and supportive ones. The emotional domain included elements of becoming heard and building confidence. In the communicative one, the main features were mutual information sharing, finding words to describe the chaos and reconstructing harmony. The supportive domain included encouragement and giving support. Experiences on non-shared agency were also found.

Conclusion: Many experiences on shared agency were found. Joint planning and working with life goals were, however, seldom reported. This result, together with experiences of non-shared agency, sets a challenge for mental health services.

Objetivo: El concepto de responsabilidad compartida se usa aquí para examinar la colaboración entre los usuarios y los profesionales. El objetivo es saber que clase de experiencias en este aspecto tienen los usuarios de los servicios de salud mental.

Método: Se entrevistaron diecinueve usuarios. Los datos se basaron en aquellas partes de las entrevistas que exploraban los siguientes aspectos: metas compartidas, ser oído, colaboración, planificación conjunta y toma de decisiones. Se aplicó un método típico de análisis inductivo de contenido.

Resultados: Se encontraron tres campos de responsabilidad compartida: emocional, comunicativo y de apoyo. El emocional incluyó elementos de ser oído y de autoconfianza. En el comunicativo las principales características fueron: información compartida, encontrar las palabras para describir el caos, y la reconstrucción de la armonía. El campo de apoyo incluyó: dar ánimo y apoyo. También se encontraron experiencias en las cuales no hubo responsabilidad compartida.

Conclusión: Se encontraron muchas experiencias de responsabilidad compartida, pero muy pocas de planificación conjunta y de trabajo orientado hacia metas en la vida. Este resultado junto con las experiencias de no-responsabilidad compartida constituye un reto para los servicios de salud mental.

Objectif: Le concept de ‘démarche partagée’ [de soin] est utilisé en référence à la collaboration entre usagers et professionnels. Cette étude a pour objectif de repérer quels types d’expérience les usagers des services de santé mentale ont de la ‘démarche partagée’.

Méthode: L’étude s’appuie sur des entretiens avec 19 usagers. Les données sont composées des extraits des entretiens qui traitaient des buts communs, d’être entendu, de collaboration et de planification jointe ainsi que du processus de décision. Des méthodes caractéristiques d’analyse inductive des contenus ont été appliquées.

Résultats: Trois domaines de ‘démarche partagée’ ont été identifiés: émotionnel, communicationnel et de soutien. Le domaine émotionnel inclut des éléments autour de l’écoute et de la construction de la confiance. Dans le domaine communicationnel, les points essentiels étaient l’échange mutuel d’information, le fait de trouver les mots pour décrire le chaos et la capacité à restaurer l’harmonie. Le domaine du soutien inclut l’encouragement et le soutien. Des expériences de ‘démarche non-partagée’ ont également été mises au jour.

Conclusion: Beaucoup d’exemples de ‘démarche partagée’ ont été trouvés. Cependant, la planification en commun et le travail sur les objectifs de vie ont été peu mentionnés. Ce résultat, avec celui des expériences de ‘démarche non-partagée’, constitue un défi pour les services de santé mentale.

Obiettivo: Il concetto di agentica condivisa viene utilizzato nell'esaminare la collaborazione tra l'utente del servizio e i professionisti. Lo studio si propone di scoprire il tipo di esperienze degli utenti di servizi di salute mentale nell’uso agentiche condivise.

Metodo: Lo studio è stato basato sulle interviste a diciannove utenti dei servizi. Il set di dati è stato formato da quelle parti delle interviste che trattatavano di obiettivi condivisi, essere ascoltati, collaborazione e pianificazione congiunta e processo decisionale. Sono stati applicati metodi tipici dell'analisi del contenuto induttivo.

Risultati: Sono stati trovati tre domini dell’agentica condivisa: quello emotivo, comunicativo e di sostegno. Il dominio emotivo include elementi di essere sentito e costruzione della fiducia. In quello comunicativo, le principali caratteristiche sono informazione reciproca condivisa, la scoperta di parole per descrivere il caos e la ricostruzione dell'armonia. Il dominio di sostegno include l’incoraggiamento e il supporto. Sono state trovate anche esperienze sull’agentica non condivisa.

Conclusione: Sono state trovate molte esperienze su agentica condivisa. Pianificazione congiunta e lavoro su obiettivi di vita sono stati, tuttavia, segnalati raramente. Questo risultato, insieme alle esperienze di agentica non condivisa, imposta una sfida per i servizi di salute mentale.
