‘A shot of his own’: The acceptability of a male hormonal contraceptive in Indonesia

Abstract

Male hormonal contraception has been shown to confer reversible infertility for at least one year; however, while clinical trials refine hormonal regimens, their acceptability, cultural meanings, and implications for study of men's sexualities remain under examined. This paper presents findings from interviews conducted with men and their female partners in a male hormonal contraception clinical trial in Jakarta and Palembang, Indonesia, and describes the ideas expressed about this new contraceptive technology. Fourteen men and their partners in Jakarta and ten men and their partners in Palembang were interviewed about their motivations to participate in the trial and their perceptions of the injection's physical, psychological and social effects. Concerns such as excess fertility and attendant economic liability shaped one quarter of motivations reported by men, while many women highlighted how a male method could help them prevent pregnancy without physical complications. Intimacy and sexual relations between couples were key themes within interpretations of contraceptive acceptability. Taken together, the narratives presented in this study call attention to the need for more nuanced analyses of contraceptive acceptability, as well as to the importance of studies of new male contraceptives for the understanding of masculinities.

Il a été démontré que la contraception hormonale masculine provoque une infertilité réversible pendant au moins un an ; cependant, alors que les régimes hormonaux sont affinés par les essais cliniques, leur acceptabilité, leurs significations culturelles, et les implications pour la recherche des sexualités masculines sont encore peu explorées. Cet article présente les résultats d’entretiens menés avec des hommes et leurs partenaires féminines au cours d’un essai clinique sur la contraception hormonale masculine à Djakarta et à Palembang en Indonésie ; et il décrit les idées exprimées à propos de cette nouvelle technologie contraceptive. Quatorze hommes et leurs partenaires à Djakarta, et dix hommes et leurs partenaires à Palembang ont été interrogés sur les motivations de leur participation à l’essai et sur leurs perceptions des effets physiques, psychologiques et sociaux de l’injection administrée. Des préoccupations telles qu’une fertilité excessive et la responsabilité économique qui lui est associée ont influencé un quart des motivations déclarées par les hommes, alors que beaucoup de femmes ont souligné combien une méthode de contraception masculine pourrait les aider à éviter des grossesses sans les exposer à des complications physiques. L’intimité et les rapports sexuels dans le couple ont été des thèmes clé des interprétations de l’acceptabilité de la contraception. Mis ensemble, les récits présentés dans cette étude attirent l’attention sur le besoin en analyses plus nuancées sur l’acceptabilité, ainsi que sur l’importance des études sur les nouveaux contraceptifs masculins, pour la compréhension des masculinités.

Resumen

Se ha demostrado que la contracepción hormonal masculina produce infertilidad reversible como mínimo durante un año. Sin embargo, aunque los ensayos clínicos definen regímenes hormonales, aún está por analizar si va a ser aceptada, sus significados culturales e implicaciones para el estudio de la sexualidad masculina. En este ensayo presentamos los resultados de entrevistas realizadas con hombres y sus compañeras en un ensayo clínico de contracepción hormonal masculina en Yakarta y Palembang, Indonesia. Asimismo, se describen las ideas expresadas sobre este nuevo método anticonceptivo. Fueron entrevistados catorce hombres y sus compañeras en Yakarta y otros diez en Palembang también junto a sus compañeras y se les preguntó qué les motivaba a participar en el ensayo y qué opinaban de los efectos físicos, psicológicos y sociales de las inyecciones. Los hombres expresaron que un cuarto de las motivaciones fueron estar preocupados por un exceso de fertilidad y la responsabilidad económica vinculada con ella, mientras que muchas mujeres destacaron que un método anticonceptivo masculino podría ayudarles a evitar un embarazo sin padecer complicaciones físicas. La intimidad y las relaciones sexuales entre las parejas fueron los temas clave en las interpretaciones a la hora de aceptar los métodos anticonceptivos. En su conjunto, por los relatos presentados en este estudio se deduce que es necesario realizar un análisis más exhaustivo sobre la aceptación del nuevo anticonceptivo y que hacen falta estudios de nuevos métodos anticonceptivos masculinos para poder entender el aspecto de la masculinidad.
