Science, technology, power and sex: PrEP and HIV-positive gay men in Paris

Abstract

The pre-exposure prophylaxis (PrEP) drug Truvada is a new HIV prevention technology that is predominantly promoted as relevant to HIV-negative gay men. This paper explores what PrEP represents for HIV-positive gay men living in Paris, based upon data collected through interviews and ethnographic research. While HIV-positive gay men do not directly consume Truvada through PrEP, they nonetheless hold opinions and understandings of this drug, specifically as it relates to their own sexuality. This paper expands the representations and meanings of this new technology in a different light through the voices of gay men living with HIV in Paris. The main argument of this article is that PrEP as an additional HIV prevention tool blurs the lines between science, technologies and human sexuality.

Résumé

La prophylaxie pré exposition (PrEP) par le médicament appelé Truvada® est une nouvelle stratégie de prévention du VIH principalement promue comme ayant une pertinence pour les hommes gays séronégatifs au VIH. Cet article explore les représentations de la PrEP chez les hommes gays séropositifs au VIH vivant à Paris, à partir de données collectées au cours d’entretiens et de celles d’une recherche ethnographique. Alors que les hommes gays séropositifs ne prennent pas le Truvada® dans le cadre de la PrEP, ils ont néanmoins des opinions et des perceptions sur ce médicament, en particulier lorsqu’il se rapporte à leur propre sexualité. Cet article élargit le champ des représentations et des significations de cette nouvelle technologie dans une perspective différente, à savoir celle des hommes gays vivant avec le VIH à Paris. L’argument principal de cet article, selon lequel la PrEP est un outil de prévention du VIH additionnel, brouille les limites entre la science, les technologies et la sexualité humaine.

Resumen

La profilaxis antes de la exposición al VIH con el medicamento Truvada es un nuevo método de prevención dirigido sobre todo a hombres homosexuales no seropositivos. A partir de datos recogidos en entrevistas y estudios etnográficos, en este artículo analizamos qué significa la profilaxis antes de la exposición para los hombres homosexuales seropositivos de París. Aunque los hombres homosexuales seropositivos no utilizan directamente Truvada como profilaxis antes de la exposición, tienen sus opiniones y conocimientos sobre este medicamento, en concreto en lo que se refiere a su propia sexualidad. En este artículo se amplían las representaciones y los significados de este nuevo método desde una perspectiva diferente a través de las voces de hombres homosexuales seropositivos de París. El principal argumento de este artículo es que la profilaxis antes de la exposición al VIH es una herramienta adicional de prevención que difumina las líneas entre la ciencia, las tecnologías y la sexualidad humana.

1. The PROUD study took place in England, with 539 men ‘who have had unprotected anal intercourse within the last 90 days and intend to do so again’ (Belluz Citation2014, 2). The iPrEX study took place in various countries, including Brazil, Thailand and South Africa, and involved 2499 high-risk men who have sex with men, and it was the first PrEP trial to publish results, in 2010, demonstrating that ‘once daily [Truvada use] provided a 44% reduction in HIV incidence among MSM [men who have sex with men]’ (Al-Tayyib et al. Citation2014; S340; see also Arnold et al. Citation2012; Hankins, Macklin, and Warren Citation2015). The Partners PrEP trial involved 4758 heterosexual serodiscordant couples in Kenya and Uganda (Baeten et al. Citation2012; Wood Citation2012) and the FEM-PrEP was a trial on PrEP and African women (Van Damme et al. Citation2012).
