‘You’ll always stay right’: understanding vaginal products and the motivations for use among adolescent and young women in rural KZN

Abstract

The use of vaginal products may increase the risk of HIV infection by affecting the vaginal biome. Understanding what vaginal products young women are using, and why, is key to assessing the complexity of sexual health and risk. This study reports on findings from research with adolescent and young women in rural KwaZulu-Natal about the vaginal products they use and motivations for using them. The study identified over 26 products that young women used to enhance their sexual experience and found some young women spent time preparing and sourcing vaginal products in order to pleasure and retain partners. Opinions differed about vaginal product use. While some women perceived that vaginal products could provide a means of out-performing other women, retaining a partner and providing sexual autonomy, there was a stigma attached to using them. Study findings highlight the social value of using vaginal products, especially in settings where partner retention is linked to economic survival. Expanding our understanding of what products are used and the reasons young women use them warrants continued investigation.

Résumé

L’utilisation de produits vaginaux peut augmenter le risque lié au VIH en affectant l’écologie vaginale. Pour évaluer la complexité de la santé sexuelle et des risques sexuels, il est essentiel de comprendre quels produits vaginaux sont utilisés par les jeunes femmes, et pourquoi celles-ci les choisissent. Cette étude rend compte des résultats d’une recherche, conduite parmi des adolescentes et des jeunes femmes dans le Kwazulu-Natal rural, sur les produits vaginaux qu’elles utilisent et leurs motivations à le faire. L’étude a identifié 26 produits utilisés pour optimiser les rapports sexuels, et révélé que les jeunes femmes consacraient du temps à préparer et à se procurer ces produits afin de satisfaire et de retenir leurs partenaires. Les points de vue sur l’usage des produits vaginaux divergeaient. Bien que certaines femmes aient perçu les produits vaginaux comme pouvant représenter un moyen de surpasser d’autres femmes, retenir un partenaire et offrir une autonomie sexuelle, le stigma lié à l’usage de ces produits était une préoccupation. Les résultats de l’étude mettent en lumière la valeur sociale de l’utilisation des produits vaginaux, en particulier dans les environnements où la rétention des partenaires est souvent liée à la survie économique. L’amélioration de notre compréhension des produits utilisés et des raisons pour lesquelles les jeunes femmes les utilisent justifie la poursuite des recherches sur ce thème.

Resumen

El uso de productos vaginales puede aumentar el riesgo de infección del virus del sida al verse afectado el bioma vaginal. Para poder evaluar la complejidad de la salud y los riesgos sexuales es fundamental entender qué tipo de productos vaginales utilizan las mujeres jóvenes y por qué los utilizan. En este estudio informamos sobre los resultados de una investigación con mujeres adolescentes y jóvenes en zonas rurales de KwaZulu-Natal sobre qué productos vaginales utilizan y los motivos de hacerlo. En el estudio se identificaron más de 26 productos que utilizaban las mujeres jóvenes para aumentar su experiencia sexual y se observó que algunas dedicaban tiempo a preparar y obtener productos a fin de dar placer y conservar a sus parejas. Las opiniones sobre el uso de productos vaginales eran muy variadas. Si bien algunas mujeres percibían que los productos vaginales podían suponer un método para ser mejores que otras mujeres, conservar a la pareja y ofrecer autonomía sexual, existía un estigma vinculado a su uso. Los resultados del estudio ponen de relieve el valor social de utilizar productos vaginales, sobre todo en entornos en los que conservar a la pareja está muchas veces relacionado con la supervivencia económica. Es preciso una investigación continuada para conocer mejor qué productos utilizan las mujeres jóvenes y por qué.

2. Ntsu is a commercially obtained tobacco inserted intra-vaginally or ingested. It is perceived to tighten and heat up the vagina.

3. Amoeba/Jelly-fish is obtained from the ocean and mixed with sugar and black tea. It is perceived to tighten and act as a love potion.

4. Commercially obtained powder inserted into vagina or placed under the tongue. It is perceived to have a drying effect on the vagina.

5. Stoney (ginger flavoured soft drink) mixed with black Halls mints (menthol throat lozenges). Ingested orally. It is perceived to have a heating and drying effect.
