Health care, social care or both? A qualitative explorative study of different focuses in long-term care of older people in France, Portugal and Sweden

Sjukvård, social omsorg eller både ock? En kvalitativ explorativ studie av olika fokus i särskilda boenden för äldre i Frankrike, Portugal och Sverige

Abstract

Challenges with an ageing population are increasingly becoming a reality in the Western world. Since cognitive impairment increases with age, we can expect an increasing number of older people in need of care. The aim of this article is to describe, analyse and compare different focuses on care of older people with dementia, using examples from France, Portugal and Sweden. The questions are principally focused on the participants’ view about their tasks, the organisation of work, the professional role and cooperation with other professions. Everyday care was studied through observations and participant observations and the staff's opinion was explored by means of interviews. Twenty-two care settings for older people were included. The findings showed that France provided mainly ‘health care’, Sweden ‘social care’ and Portugal an integrated ‘health care and social care’. In a comparative perspective the Portuguese general care of older people, which focuses on integration of health care, social care and social work, also seems to provide care for older people suffering from dementia which best corresponds to the previously developed group living model.

Stora utmaningar med en åldrande befolkning blir mer och mer en realitet i den industrialiserade delen av världen. Då kognitiva nedsättningar ökar med åldern kan vi förvänta oss ett ökande antal äldre i behov av vård och omsorg. Genom att använda exempel från Frankrike, Portugal och Sverige är syftet med denna artikel att beskriva, analysera och jämföra olika fokus på vård och omsorg av gamla demenssjuka människor. Särskilt belyses frågor kring deltagarnas syn på sina arbetsuppgifter, organisering av arbetet, deras professionella roll och samarbete med andra yrkesgrupper. Vardagen i vården följdes genom observationer och deltagande observationer och personalens uppfattningar studerades genom intervjuer. I studien ingick 22 boenden för äldre. Resultaten visar att man i de studerade enheterna i Frankrike huvudsakligen bedrev ‘sjukvård’, i Sverige ‘social omsorg’ och i Portugal en ‘integrerad sjukvård och social omsorg’. I ett jämförande perspektiv kan man konstatera att den generellt inriktade äldreomsorgen i Portugal, med fokus på en integrering av såväl sjukvård och social omsorg som socialt arbete, bäst tycks stämma överens med de tidigare utvecklade formerna av gruppboende för äldre människor med demenssjukdomar.
