Awaiting knee joint replacement surgery: An occupational perspective on the experience of osteoarthritis

ABSTRACT

Total knee replacement (TKR) is an increasingly common surgery among individuals with osteoarthritis (OA). Although the outcomes of this surgery are generally deemed positive, many are not fully satisfied with their return to pre-TKR occupations. The literature on pre-TKR occupational engagement is limited and primarily describes how the symptoms associated with OA restrict participation in occupation with little attention given to the significance occupations hold for individuals waiting for a TKR. A secondary analysis of eight purposively extracted transcripts from a primary study examining why people with OA do or do not re-engage in pre-operative occupations following hip or knee replacement was conducted. A qualitative thematic analysis approach was used to examine the pre-TKR experiences of these individuals from an occupational perspective. Three themes were identified: (1) living with OA: it is more than just the knee, (2) gradually losing my occupational identity, and (3) losing too much—a “tipping point.” The findings suggest that the significance of lost occupations is key and unique to each individual and affects their decision for surgery.

El reemplazo total de rodilla (RTR) es un procedimiento quirúrgico cada vez más solicitado por individuos aquejados de osteoartritis (OA). Aunque en relación con este tipo de cirugía generalmente se verifican resultados favorables, muchos pacientes reportan no quedar enteramente satisfechos al momento de retomar las ocupaciones que tenían previamente al RTR. Existen pocos estudios que abordan la participación ocupacional previa al RTR; los mismos tienden a describir cómo los síntomas asociados a la OA restringen la participación en la ocupación, y se enfocan escasamente en el significado que las ocupaciones tienen para quienes están a la espera de un RTR. Debido a ello se realizó un análisis secundario de ocho transcripciones seleccionadas intencionalmente de un estudio primario que examinó el motivo por el cual las personas con OA vuelven a participar (o no) en sus ocupaciones anteriores a la cirugía para reemplazar la cadera o la rodilla. Con este propósito se empleó un enfoque de análisis temático cualitativo para examinar, desde una perspectiva ocupacional, las vivencias de dichas personas durante el periodo anterior a su cirugía RTR. Al respecto, se identificaron tres temas relevantes: (1) vivir con la OA: es mucho más que la rodilla; (2) la pérdida gradual de mi identidad ocupacional; y (3) la pérdida excesiva —“un punto crítico”. Los hallazgos sugieren que la importancia de las ocupaciones perdidas constituye un aspecto clave y único para cada individuo, que incide en su decisión de operarse.
