Transgender experiences of occupation and the environment: A scoping review

ABSTRACT

Background: ‘Cisnormativity’ refers to the Western cultural belief that gender (man/woman) mirrors sex (male/female). This review considers the occupational experiences of people whose gender identity differs from what is expected based on their sex at birth, gender identities operationalised as ‘transgender’. Method: A scoping review was conducted to examine the subjective experiences of occupation for transgender people and the enabling and/or restrictive features of the environment. Forty-one studies were reviewed, using inductive and deductive thematic analysis methods. Results: Three major themes relating to occupational experiences were identified: a) doing difference: gender expression and transition, b) recognisably different: macroaggressions and transphobia, and c) responding to difference: adaptation and identity affirmation. Environmental barriers were reported at a higher frequency than enabling features and pertained to social, physical, cultural and institutional factors. Conclusions: While occupation enables expression of transgender identities, cisnormative ideology shapes participatory norms in Euro-Western contexts. Occupational opportunities are unequal for transgender people, with the policing of binary gender restricting opportunities for inclusion and participation. This review functions as a critical starting point for broader conceptual thinking on occupation, resistance and identity-management for people who are transgender.

背景：“顺式规范性”是指这样的西方文化理念，即，性别（男人/女人）直接反映性（男性/女性）。本报告考虑了性别认同与出生性别不同的人的生活活动经历，他们的性别认同被归为“变性人”。方法：对变性人的主观生活活动体验和环境的促成和/或限制性特征进行范围界定审查。回顾41项研究，采用归纳和演绎的主题分析方法。结果：确定了与生活活动经历相关的三个主要主题：a）行为差异：性别表达和转变；b）明显不同：严重侵犯和恐变性心理；c）对差异的反应：适应和身份确认。环境障碍的报告频率高于使能特征，并与社会、物理、文化和体制因素有关。结论：虽然生活活动可以表达变性身份，但CIS规范性意识形态在欧美背景下形成了参与性规范。变性人的生活活动机会是不平等的，二元性别的政策限制了融入和参与的机会。该报告是对变性人的生活活动、毅力和身份认同进行更广泛概念性思考的一个关键起点。

Antecedentes: el término “cisnormatividad” hace referencia a la creencia cultural occidental de que el género (hombre/mujer) refleja el sexo (hombre/mujer). La presente revisión aborda las experiencias ocupacionales de personas cuya identidad de género difiere de lo que se espera de ellas en función de su sexo al nacer, esto es, de identidades de género operativizadas como “transgénero”. Método: se realizó una revisión de alcance para examinar las experiencias subjetivas de ocupación vivenciadas por personas transgénero, además de las características habilitantes y/o restrictivas del entorno. Con este objetivo se revisaron 41 estudios, utilizando metodologías de análisis temático inductivo y deductivo. Resultados: se identificaron tres temas principales relacionados con las experiencias ocupacionales: a) diferenciación: expresión y transición de género, b) diferencias reconocibles: macroagresiones y transfobia, y c) respuesta a la diferencia: adaptación y afirmación de identidad. La presencia de barreras en el entorno se reportó con mayor frecuencia que la de características habilitantes; dichas barreras tenían que ver con factores sociales, físicos, culturales e institucionales. Conclusiones: Mientras la ocupación permite la expresión de identidades transgénero, la ideología cisnormativa moldea las normas de participación en los contextos eurooccidentales. En este sentido, las oportunidades ocupacionales para las personas transgénero son desiguales, ya que la vigilancia de género binario restringe las oportunidades de inclusión y participación. Esta revisión constituye un punto de partida crítico hacia un pensamiento conceptual más amplio sobre la ocupación, la resistencia y el manejo de la identidad de las personas transgénero.
