Becoming a mother in the neonatal intensive care unit: A narrative review

ABSTRACT

Rationale: Only a small proportion of mothers who delivered babies admitted to a Neonatal Intensive Care Unit (NICU) were able to hold them within the first hour following birth. For the majority, this separation left them feeling responsible for their infant’s ill health and powerless in their recovery. Exploring how these feelings and becoming a mother in the NICU impacts the occupational role of mothering may assist with better supporting mothers during their infant’s NICU stay and following discharge. Objective: To examine mothers’ lived experiences of their infant’s NICU stay and how it may impact their occupational role of mothering. Method: Six online databases (CINAHL, PubMed, PsycINFO, Gender Studies Database, Sociological Abstracts, Social Services Abstracts) were searched using key words, retrieving 352 articles. A total of 12 studies were retained for appraisal after screening title, abstract, and full text reviews. Findings: Thematic analysis identified three themes: 1) struggling to claim the maternal role, 2) lack of psychological readiness, and 3) seeking connection with one’s infant. Conclusions: This review offers an occupational perspective to the disruption new mothers experience as the result of the NICU environment and psychological distress that might limit their ability to fulfill mothering occupations such as breastfeeding. This shift in perspective may support women to engage more fully in the occupations associated with becoming a mother and adapting into the new situation.

Justificación: Sólo una pequeña proporción de las madres que dieron a luz bebés que debieron ser ingresados en una Unidad de Cuidados Intensivos Neonatales (UCIN) pudo sostenerlos dentro de la primera hora después del nacimiento. Esta separación ocasionó que la mayoría de ellas se sintiera responsable de la mala salud de su bebé e impotente para apoyar su recuperación. Explorar cómo estos sentimientos y el hecho de convertirse en madre en la UCIN influyen en el papel ocupacional de la maternidad puede contribuir a brindar un mejor apoyo a las madres durante la estancia de su bebé en la UCIN y una vez que son dados de alta. Objetivo: Examinar las vivencias experimentadas por las madres durante la estancia de su bebé en la UCIN y cómo estas pueden afectar su papel ocupacional como madres. Método: Se realizaron búsquedas en seis bases de datos en línea (CINAHL, PubMed, PsycINFO, Gender Studies Database, Sociological Abstracts, Social Services Abstracts) utilizando palabras clave, obteniéndose 352 artículos. Se seleccionaron 12 de estos estudios con el fin de evaluarlos después de revisar el título, el resumen y los textos completos. Hallazgos: El análisis temático identificó tres ejes: 1) la lucha por reclamar el papel de madre, 2) la falta de preparación psicológica, y 3) la búsqueda de conexión con el bebé propio. Conclusiones: Esta revisión aporta una perspectiva ocupacional a la perturbación experimentada por las nuevas madres como resultado del ambiente de la UCIN y la angustia psicológica que podría limitar su capacidad para cumplir con las ocupaciones de la maternidad, por ejemplo, la lactancia. Este cambio de perspectiva puede ayudar a las mujeres a participar más plenamente en las ocupaciones asociadas con la maternidad y a adaptarse de mejor manera a la nueva situación.
