The production of this Supplement has been funded by the US Agency for International Development. However, in strict accordance with the policies and procedures of the journal, the funder had no involvement in the editorial process. All author affiliations are clearly expressed and the usual conflict of interest statements have been required. All submitted manuscripts were subjected to stringent peer review by independent volunteer reviewers who were not suggested by the authors and, for the most part, accepted manuscripts were revised in accordance with reviewer suggestions. Where reviewers offered incompatible comments or opinions, decisions regarding acceptance and revision were made by the editorial staff of the journal. Every precaution was taken to maintain the scientific integrity of the Supplement and a total separation between the journal and its funding source was maintained at all times throughout the process.
