The authors would like to report an additional methodological limitation not described in the article.
Because of the skip patterns of computer-assisted phone interviews in the 2013 Childhood National Immunization Coverage Survey, parents of children and adolescents who had not received any vaccine in their lifetime were not asked about knowledge, attitudes and beliefs (KAB) or about vaccine refusal.
The effect on rates and associations reported in Tables 2 and 3 is most likely small in magnitude: girls never vaccinated represented 3.6% (54/1490) of those not vaccinated for HPV and < 1% (54/5720) of the analytical sample. However, associations between KAB and HPV refusal may have been biased toward the null by the exclusion of those parents most adverse to vaccines.
Rates of non-vaccination and associations described in Table 1 of the article are not affected at all.
