On four occurrences, Figure 16 was incorrectly referred to as Figure 14 within the text on pages 61, 62, and 63.

On page 61, the first sentence in the second paragraph should read, “For Event A in Figure 16, only the Fp1-F7 and Fp2-F8 channels need to be considered in determining the direction of eye movement; note that the deflection is downwards.”

On page 62, the second sentence in the first paragraph should read, “For Event C in Figure 16, the F7 and F8 electrodes are recording opposite polarities: F7 is negative and F8 is positive.”

On page 62, the sixth sentence in the first paragraph should read, “If attention is given to just F7 and F8 in Event C of Figure 16, the direction of eye movement can be determined: Event C is associated with a more positive F8 but a more negative F7 electrode.”

On page 63, the second complete sentence in the first paragraph should read, “For Event D in Figure 16, the waveform behavior is opposite to that of Event C.”
